Join our innovative Hospital at Home (H@H) service and play a leading role in delivering acute hospital-level care in patient's own homes across Bath & North East Somerset, Swindon and Wiltshire. This is a unique opportunity to move beyond the traditional consultant role and influence care at a system wide level. As the senior clinical lead for H@H, you will drive service development, provide expert frailty leadership, support complex clinical decision making, and help create new pathways that reduce hospital admissions and promote home first care. Working with a highly skilled multidisciplinary team, you will champion innovative models of care that improve outcomes and patient experience. Working closely with multidisciplinary teams, operational leaders and system partners, you will help shape clinical pathways, support complex decision-making, champion best practice in frailty care, and drive service improvement. You will play a key role in strengthening partnerships across acute, community, primary care and urgent care services, ensuring integrated, patient centered care that supports admission avoidance and home first approaches. The role also includes leading clinical governance, quality improvement, workforce development and strategic service transformation to ensure Hospital at Home continues to deliver safe, effective and sustainable care for older people. Please see the attached for the full job description for a full list of job responsibilities.Ideal Candidate: Essential MRCP (UK) or equivalent Entry on the GMC One of CCT, CESR, European community rights in Geriatric Medicine Extensive medical experience at registrar or consultant level Knowledge and understanding of clinical governance Experience of working with managers to improve patient experience andservice delivery Experience in clinical audit Understanding of clinical research Proven ability to motivate, support and work within a multi-disciplinary team High professional and ethical standards Excellent written and verbal communication skills A commitment to continuous professional development Good IT skills Ability to balance competing pressures and proven ability to organise and priorities workload, delegating as appropriate Desirable Relevant higher degree (MD/PhD) Qualification in Teaching & Learning Experience working across organisational boundaries. Special interest or advanced experience in community geriatrics Involvement in service developmentand enacting change Ability to lead clinical services Involvement in research Experience in teaching practically a multi-professional team Advanced leadership skills Other requirements: Full UK driving license with access to a vehicle for work purposes. Willingness to work across different Trust areas as required. Flexible availability to meet service demands.
