Summary

NURSE TEAM LEADER PRACTICE NURSING TEAM Hours: 30 hours per week ( upto 37.5) Location: Ancora Medical Practice across both practice sites Responsible to: Practice Directors / GP Partners Reports to: Practice Director / Lead GP for Nursing Team Professional accountability: NMC Code of Professional Conduct and relevant professional standards JOB PURPOSE The Nurse Team Leader will provide clinical leadership, coordination and professional support to the practice nursing team, ensuring that patients receive safe, effective and consistent care across both practice sites. The postholder will take a lead role in staff development, training, clinical standards, infection prevention and control, and the ongoing development of the nursing team. Working closely with the Practice Director, clinicians and HCA Team Leader, the Nurse Team Leader will help ensure that the nursing service is well organised, appropriately staffed and continually improving. The successful candidate will ideally have strong diabetes experience and be able to support the practice in developing and strengthening its diabetes service. KEY RESPONSIBILITIES 1. Clinical Leadership Provide day-to-day clinical leadership and professional support to the nursing team. Promote high standards of patient care, safety and professionalism. Support nurses and nursing associates with clinical queries and day-to-day practice. Ensure agreed clinical protocols, guidelines and pathways are followed. Identify areas where clinical practice requires improvement and work with staff to address these. Support consistent clinical standards across both practice sites. Work closely with the Lead Gp and Practice Director on clinical workforce planning. Contribute to clinical meetings, audits and service development. 2. Staff Development and Training Take overall responsibility for coordinating the development and training programme for nursing staff and HCAs. This will include: Maintaining an up-to-date clinical staff training matrix. Identifying individual training and development needs. Ensuring staff maintain the competencies required for their roles. Supporting induction and competency assessment for new clinical staff. Coordinating appropriate clinical training and updates. Monitoring mandatory and role-specific training. Identifying opportunities for staff to develop additional skills and responsibilities. Supporting staff undertaking further qualifications, including prescribing and specialist courses. Working with individual staff members to develop appropriate professional development plans. Ensuring training and competency records are maintained and available for audit/CQC purposes. Areas of clinical competency should include, as appropriate: Diabetes COPD Asthma Vaccination and immunisation Cervical screening Wound care Insulin initiation Long-term condition management Independent prescribing LARC and other specialist areas Basic and advanced clinical skills relevant to the practice 3. Diabetes Leadership The postholder will ideally have significant experience in diabetes management and will take a lead role in supporting the practice's diabetes service. Responsibilities will include: Supporting the development of the practice diabetes service. Reviewing clinical processes and identifying areas for improvement. Supporting nurses and other clinical staff in diabetes management. Promoting evidence-based diabetes care. Supporting appropriate recall and monitoring systems. Reviewing performance and identifying gaps in care. Supporting clinical audits and quality improvement. Providing education and advice to colleagues where appropriate. Working with GPs and other members of the multidisciplinary team to improve patient outcomes. 5. Clinical Rooms, Equipment and Stock Work with the HCA Team Leader to ensure clinical rooms are appropriately stocked and maintained. Establish appropriate stock levels for clinical areas. Monitor expiry dates and equipment checks. Ensure equipment is maintained, calibrated and fit for purpose. Identify and report deficiencies promptly. Support the development of clear systems for stock ordering and monitoring. Ensure clinical areas remain clean, organised and professionally presented. 6. Governance, Audit and Quality Improvement Participate in clinical audits and quality improvement projects. Support the practice in meeting CQC requirements. Ensure appropriate evidence is maintained for training, competency and clinical governance. Monitor incidents and significant events relevant to nursing services. Contribute to learning from incidents and complaints. Help develop and review clinical protocols and procedures. Identify risks within the nursing service and escalate these appropriately. Support implementation of changes arising from audits, inspections and clinical governance reviews. 7. Staff Supervision and Support Provide regular support and professional supervision to members of the nursing team. Encourage a positive, respectful and supportive working environment. Promote good communication between nurses, HCAs, GPs and wider practice staff. Identify concerns regarding performance, conduct, capability or training and raise these with the Practice Manager appropriately. Support the Practice Manager with clinical aspects of staff induction, development and competency. Encourage staff to take responsibility for their own professional development. The Nurse Team Leader will not replace the Practice Manager's responsibility for formal HR, disciplinary or capability procedures, but will provide appropriate clinical information and support where required. 8. Team Organisation Work with the Practice Director to ensure appropriate clinical staffing across both sites. Assist with planning annual leave and clinical cover where appropriate. Support effective use of nursing skills and competencies. Help ensure the nursing team works efficiently and consistently. Identify service pressures and escalate these appropriately. Support the development of new ways of working. 9. Communication The postholder will be expected to communicate effectively with: Practice Directors Senior Managers GPs Practice nurses Nursing associates HCAs Administrative and reception teams PCN colleagues Community and secondary care colleagues Other external healthcare professionals The Nurse Team Leader will be expected to promote a culture of professionalism, teamwork, accountability and continuous improvement. PERSON SPECIFICATION Essential Registered Nurse with current NMC registration. Significant experience working within general practice/primary care. Demonstrable clinical experience in long-term condition management. Strong organisational and leadership skills. Experience supporting or supervising other clinical staff. Ability to identify training and development needs. Understanding of infection prevention and control. Understanding of clinical governance, audit and quality improvement. Ability to work independently and make appropriate clinical decisions. Excellent communication and interpersonal skills. Ability to work across both practice sites. Commitment to maintaining high standards of patient care. Desirable Significant experience in diabetes care. Independent prescribing qualification. Experience of insulin initiation. Experience of supporting staff competency assessments. Experience of infection prevention and control leadership. Experience of clinical audits and quality improvement. Experience of working in a leadership or senior nursing role. Experience of supporting CQC preparation and compliance. Additional specialist qualifications relevant to primary care. KEY OBJECTIVES FOR THE FIRST 612 MONTHS The Nurse Team Leader will be expected to: Complete a full review of the nursing team's skills and competencies. Establish and maintain a comprehensive clinical training matrix. Identify gaps in mandatory and clinical training. Develop an annual nursing team training and development plan. Review diabetes services and identify opportunities for improvement. Establish a structured programme of clinical audits. Review infection prevention and control arrangements across both sites. Ensure clinical rooms, equipment and stock arrangements meet agreed standards. Support consistent clinical practice across both sites. Establish clear communication and accountability within the nursing team. Work with the Lead GP and Practice Director to develop the nursing service. Support the practice in maintaining high standards of clinical governance and CQC compliance. THE ROLE This is an opportunity for an experienced practice nurse to take a key leadership role in a large, busy GP practice, helping shape the future development of the nursing team and the services provided to patients. The successful candidate will have the opportunity to develop the role and make a significant contribution to staff development, clinical quality, diabetes care, infection control and the wider development of primary care services.
