Summary

As a Pharmacy Technician, you will work under the supervision of the Pharmacy team, and GPs to support the delivery of safe, effective and patient-centered care across the practice. Your responsibilities will include: Supporting medicines optimisation and prescribing safety initiatives. Undertaking medication reviews and medicines reconciliation. Processing incoming letters relating to medication. Managing repeat prescribing processes and responding to medication queries. Supporting high-risk medicines monitoring, including Direct Oral Anticoagulant (DOAC) optimisation. Completing clinical searches, audits and quality improvement projects. Supporting delivery of QOF, Enhanced Services and medicines optimisation initiatives. Working collaboratively with GPs, Clinical Pharmacists, Nurses, Community Pharmacies and Secondary Care providers. Supporting care home medicines optimisation and transfer of care processes. Contributing to service development and continuous improvement across the practice. Following Infection Prevention and Control guidelines. Raising safeguarding concerns where appropriate, relating to medications or other issues.
