Summary

Key Responsibilities Clinical Assessment and Management Undertake comprehensive clinical assessments of patients presenting with undifferentiated and diagnosed conditions. Diagnose, treat, and manage acute and minor illness within the scope of professional competence. Formulate management plans and arrange appropriate follow-up care. Request and interpret diagnostic investigations where appropriate. Prescribe medications (if qualified as an independent prescriber) in accordance with current legislation and practice policies. Refer patients to appropriate healthcare professionals, secondary care services, and community services as required. Recognise clinical deterioration and escalate concerns appropriately. Triage Responsibilities Provide clinical triage of patient requests received via telephone, digital consultation platforms, and clinical workflow systems. Assess urgency and ensure patients are directed to the most appropriate clinician or service. Prioritise and manage same-day urgent care requests. Support the management of demand within the practice by signposting patients effectively. Document triage outcomes accurately and comprehensively within the clinical record. Home Visits Undertake home visits for housebound patients when required, care home residents, and patients unable to attend the surgery when clinically indicated. Assess, diagnose, treat, and manage patients within their home environment. Develop care plans and arrange follow-up support where required. Liaise with community nursing teams, social care providers, and other healthcare professionals to ensure continuity of care. Ensure appropriate safeguarding and risk assessments are considered during home visits. Long-Term Condition Management Support the monitoring and management of patients with long-term conditions. Promote self-care and patient education to improve health outcomes. Participate in proactive care planning and multidisciplinary reviews where appropriate. Safeguarding and Patient Safety Identify, assess, and respond appropriately to safeguarding concerns involving children and vulnerable adults. Report and document safeguarding concerns in line with local and national guidance. Participate in incident reporting, significant event reviews, and quality improvement initiatives. Multidisciplinary Working Work collaboratively with GPs, nurses, pharmacists, healthcare assistants, social prescribers, and other healthcare professionals. Participate in multidisciplinary team meetings and case discussions. Foster positive working relationships across primary, community, and secondary care services. Governance and Professional Development Maintain professional registration with the Health and Care Professions Council (HCPC). Participate in appraisal, revalidation, and continuing professional development activities. Adhere to practice policies, clinical governance requirements, and national guidelines. Maintain accurate, timely, and legible clinical records using the practice clinical system.
