Summary

The post holder will be responsible for:

Patient Navigation

• Proactively manage and coordinate inter-trust cancer pathways, ensuring patients are tracked, monitored, and supported throughout their cancer journey.

• Maintain oversight of patient pathways to ensure investigations, referrals, and treatments progress in a timely and efficient manner, in line with agreed SSG pathways and Cancer Waiting Times standards.

• Identify, investigate, and resolve any delays or barriers within the patient pathway at the earliest opportunity.

• Escalate unresolved issues appropriately in accordance with Trust policies, procedures, and cancer network guidance.

• Work collaboratively with clinical teams and relevant stakeholders to support seamless communication and delivery of a coordinated, patient-centred service.

Key Responsibilities

Patient Navigation

• Coordinate and manage inter-trust cancer pathways, ensuring patients are tracked, monitored, and supported throughout their journey from referral to treatment.

• Maintain oversight of patient pathways, monitoring progress and reporting performance to ensure compliance with agreed SSG pathways and Cancer Waiting Times targets.

• Identify and resolve pathway delays or barriers promptly, escalating issues in line with Trust policies and cancer network procedures.

Specialist/Local Multidisciplinary Team (S/LMDT) Coordination

• Provide administrative coordination and support for cancer S/LMDT meetings, ensuring all required clinical information, including patient records, imaging, and pathology results, is available for discussion.

• Attend meetings and maintain accurate records of attendance, discussions, and outcomes.

• Support timely and accurate transfer of patients and clinical information between organisations, following agreed network pathways and communication protocols.

Data Collection and Analysis

• Collect, maintain, and validate cancer pathway data accurately in line with local and national reporting requirements.

• Update and manage relevant cancer databases and systems, including Open Exeter and Trust Cancer Waiting Times systems.

• Monitor pathway performance, identify delays and target breaches, and provide reports to the Cancer Manager, Lead S/LMDT Clinician, and S/LMDT Service Improvement Clinical Lead.

Patient Navigation Responsibilities

• Coordinate and navigate patients through their cancer care pathway from referral to treatment, ensuring all stages are completed within the timescales agreed by the SSG. Utilise relevant hospital IT systems, including PAS, endoscopy reporting systems, radiology systems, and histology systems, to monitor and track patient progress.

• Proactively arrange and pre-book appointments in line with agreed SSG pathways, ensuring patients progress through each stage of their pathway within required timescales.

• Monitor diagnostic appointments for priority patients and escalate any delays or appointments that fall outside the required target timeframe.

• Identify, investigate, and proactively address delays or barriers at any stage of the patient pathway. Where resolution is not possible, follow agreed escalation processes and inter-trust transfer procedures, informing line management of any concerns.

• Ensure timely transfer of patients requiring tertiary centre input, following network and SSG protocols. Complete and submit SMDT referral documentation within 24 hours of the clinical decision to refer, ensuring all required clinical information is provided within agreed timescales.

• Identify pathway bottlenecks and communicate issues regularly with line managers, contributing to the development and implementation of solutions to improve patient flow.

• Liaise effectively with internal teams, partner Trusts, and network organisations to support continuous monitoring and coordination of patient pathways.

• Advise clinical teams when waiting time adjustments are appropriate, ensuring all adjustments are accurately recorded and documented in accordance with national guidance and Trust procedures.
