Summary

Key requirements of the role: Long term conditions management, assess, plan and evaluate individual treatment plans Assessing care and nursing needs for patients in their own homes Management of frail patients Assess and evaluate treatment programmes that promote health and wellbeing Support anticipatory care planning and advanced care planning discussions Work with other health care professionals to diagnose, monitor, manage and treat long term conditions, including holistic and non drug based treatment methods Wound management including compression therapy Phlebotomy Health promotion Health screening Undertaking planned and opportunistic tests and investigations at the request of other members of the clinical team Assist in and perform routine tasks related to patient care as directed. Hypertension management. Weight monitoring and support. Health promotion. First aid and management of emergencies e.g. cardiac arrest, shock, haemorrhage. Main duties and key responsibilities: Maintain patient confidentiality and work within guidelines and protocols at all time Initiate and arrange a methodical assessment of patient and service need To be aware of health and safety issues affecting staff, patients and visitors and keep up to date with health and safety training Maintain accurate records of the care given, ensuring accurate data input into computerised systems. Assist and chaperone, when necessary, with the examination and treatment of patients. Work collaboratively across the PCN to enhance integration with community, secondary and social care services Be flexible to work to current PCN and General Practice priorities Take a preventative approach to patient care Work in the community and across PCN practices as necessary, including sometimes in nursing or care homes Team working Understand own role and scope, and identify how this may develop over time Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working Delegate clearly and appropriately, adopting the principles of safe practice and assessing competence Ensure clear referral mechanisms are in place to meet patient need Prioritise own workload and ensure effective time-management strategies are embedded within the culture of the team Work effectively with others to clearly define values, direction and policies impacting upon care delivery Participate in team activities that create opportunities to improve patient care Participate in and support local projects as agreed with the practice management team Equality and diversity Identify patterns of discrimination, take action to overcome this, and promote diversity and quality of opportunity Enable others to promote equality and diversity in a non discriminatory culture Support people who need assistance in exercising their rights Monitor and evaluate adherence to local chaperoning policies Act as a role model in the observance of equality and diversity good practice Accept the rights of individuals to choose their care providers, participate in care and refuse care Assist patients from marginalised groups to access quality care This Job Description is not exhaustive and may change as the post develops.
