Summary

Family Support Deliver one-to-one healthy lifestyle appointments with children, young people and families. Deliver evidence-based nutrition education using approved service resources. Deliver parent and carer support sessions and workshops to improve knowledge, confidence and practical skills in supporting healthy family lifestyles. Deliver group sessions as part of a service development plan and use participant feedback to continuously improve programme deliver. Support families to develop personalised healthy lifestyle plans. Use motivational interviewing and behaviour change techniques. Monitor progress and review goals. Complete anthropometric measurements. Escalate complex clinical concerns to Nutritionists, GP or Clinical Lead. Identify safeguarding concerns and escalate appropriately. Nutrition Support Deliver nutrition advice within agreed competency framework. Reinforce healthy eating guidance developed by the service. Deliver practical food education. Deliver follow-up appointments under agreed protocols. Engagement & Community Delivery Represent the service at schools. Deliver assemblies and workshops. Attend community engagement events. Build relationships with schools, GP practices and community organisations. Promote referrals into the service. Support promotional campaigns. Administration & Service Support Support referral processing and booking appointments. Contact families regarding appointments. Maintain accurate electronic records. Assist with audits. Provide cover for administrative colleagues where appropriate. Develop resources. Support audits. Contribute case studies. Support service improvement projects. Skills and Competencies Required The successful candidate will demonstrate the following skills and competencies: Behaviour Change: Experience of delivering person-centred behaviour change interventions, supporting children, young people and families to achieve sustainable healthy lifestyle improvements through evidence-based approaches such as motivational interviewing and SMART goal setting. Communication: Excellent verbal and written communication skills, with the ability to adapt communication styles to engage children, young people, parents/carers, professionals and community partners effectively. Relationship Building: Ability to build positive, trusting and professional relationships with families, colleagues, schools, healthcare professionals and wider stakeholders to support collaborative working and improve health outcomes. Engagement and Facilitation: Confident in delivering engaging one-to-one appointments, group programmes, parent workshops and community sessions, creating an inclusive and supportive environment that encourages participation and behaviour change. Organisation and Time Management: Excellent organisational skills with the ability to manage a varied workload, prioritise competing demands, meet deadlines and maintain accurate clinical and administrative records. Administration and Digital Skills: Competent in using Microsoft Office and electronic patient record systems, with strong attention to detail when recording information, monitoring outcomes and completing administrative tasks. Team Working: A collaborative team player who contributes positively within a multidisciplinary team, shares knowledge and supports colleagues to deliver high-quality, family-centred care. Adaptability and Flexibility: Able to work across a variety of settings including community venues, schools, clinics and digital platforms, responding positively to changing service needs and priorities. Health Promotion: Passionate about improving the health and wellbeing of children, young people and families, with an understanding of health inequalities and the importance of delivering accessible, inclusive and culturally sensitive services. Professionalism: Demonstrates integrity, professionalism and accountability, maintaining confidentiality, safeguarding responsibilities and high standards of practice at all times. Continuous Improvement: Open to learning, reflective practice and service development, actively contributing ideas to improve service delivery, client experience and health outcomes.
