Summary

A Vacancy at Oxford University Hospitals NHS Foundation Trust.

A Clinical Fellow post is available for an initial 12 months, which may be extended by mutual arrangement. Applicants are welcomed from a background in anaesthesia at ST7 level, or post-CCT. The Fellowship will be tailored according to experience.

Successful applicants will be oncall for the NECA. They will have one day per week dedicated to complex orthopaedic anaesthesia projects and quality improvement in the NECA. It is expected that they will undertake a structured project relevant to complex orthopaedic anaesthesia. The topic will be chosen by mutual agreement with the programme supervisors (Drs Vassilis Athanassoglou and James Day) based on the Fellow’s experience and existing ideas / interests.

The posts accompany the formation of the Nuffield Orthopaedic Centre Enhanced Care Area (NECA). This year long post will allow the trainee exposure to looking after patients on the NECA and will familiarise them with complex orthopaedic anaesthesia, regional anaesthesia, and other anaesthetic subspecialties after discussion with the College Tutors.

In addition the Fellow will have ample opportunity to develop their skills in perioperative medicine, point of care ultrasound, and interested and highly qualified fellows will be given the opportunity to develop skills in echocardiography. When the fellow is not on shift in the enhanced care area they will be able to rotate through sub-specialties in Anaesthesia including but not limited to Vascular, Regional, Transplant, Major Cancer, Emergency, and Trauma Anaesthesia.

a. Aims and responsibilities:

The main responsibility is the overnight cover of the NECA. Day time responsibilities will vary depending on the needs of the anaesthetic department but will primarily be focused at the Nuffield Orthopaedic Centre. However, the post holder will work across the Oxford Radcliffe NHS Trust group of hospitals, especially in time of unprecedent demand (e.g. pandemic):

• The John Radcliffe Hospital – Trauma, Neuro, Obstetrics, Vascular, Airway, Plastics, Paediatrics, Gynaecology, Obstetrics and Intensive Care

• The Churchill Hospital – Transplant, Oncology, Airway, and Intensive Care

• The Nuffield Orthopaedic Centre – Theatres and Enhanced Care Unit

The specific aims and objectives will be set by the Fellow in conjunction with their supervisors. The Fellowship will allow development of advanced skills in perioperative medicine, and various anaesthesia disciplines including both planning and delivering anaesthesia to the most complex cases occurring in this specialist centre. There will be opportunities to work with distant, local and direct supervision.

On starting the post Fellows work under the direct supervision of senior staff until they have successfully completed appropriate induction to their new site of work. The Fellow will average three half day sessions per week of training activities supervised by a Consultant Trainer. Every three months during the post (or more frequently if required) training will be reviewed by their Supervisor.

Fellows are expected to keep accurate contemporaneous records of clinical activities or events, including pre-operative assessment, pre- and postoperative care, pain relief, critical incidents, etc., using the Trust approved documentation.

Fellows are expected to keep a record of their training including a log book of cases and a record of other activity.

The Departmental Administrator produces a weekly rota allocating each member of the trainee grades to particular duties. It is expected that the successful candidate will perform duties appropriate to the grade and to his/her level of experience. There will be times where the schedule has to be changed at short notice after the rota has been published and to move the Fellows to cover a different list, occasionally not directly related to the module they are currently undertaking.

b. Clinical:

The work schedule is resident, full shift 1:8 on call rota, mainly covering the NECA but this may occasionally change to involve cover in Obstetrics and all Critical Care, especially in period of high need, e.g. pandemic. Daytime work will typically include 3-monthly modules in specialties discussed between the Fellow, the programme director, and the College Tutors to ensure training of current trainees is not compromised.
