Summary

To act as a primary and/or associate nurse for an identified group of patients and supervise others in carrying out these roles. Ensure that all patients have an up-to- date and relevant care plan according to local policy Understand and follow agreed local and national guidance in relation to the Care Programme Approach and ensure that other staff comply with guidance To Involve and encourage patients to accept an optimum level of responsibility for their individual programmes and, with their consent, seeks the co-operation of relatives and friends where appropriate Help to maintain standards of care set by the Ward/Unit Manager. Participates in evaluation studies of projects and audits as required. Assists the Ward/Unit Manager in implementing agreed standards Support the development of good nursing practice with reference to local clinical standards and policies and the NMC Code of Professional Conduct for nurses and acquires and use skills appropriate to the care of patients who require support and special intervention Participate in the establishment, running and evaluation where required of therapeutic groups for patients and accept supervision for such work on a regular basis. Report all incidents involving patients, staff and visitors, recording them according to trust policy. When necessary to initiate post incident analysis of incidents, providing support and guidance as necessary and assist in investigating serious incidents and post-incident planning. Assist in the investigation of and response to complaints Maintains effective liaison with all members of the multi-disciplinary team and other agencies involved in the care of patients. Works as a member of multidisciplinary team, promoting confidence in the competence and skill of the nursing team. Keeps abreast with developments in own speciality of nursing and research in mental health nursing and relate findings to practice. To be fully involved in presentations in ward rounds, handovers and other meetings where patient care is discussed and reviewed. Chairing such meetings when required. To expedite and participate in the assessment, admission, transfer and discharge of patients according to local guidelines and standards. To co-ordinate the dispensing of medication according to trust policy and ensure junior staff adhere to Trust guidelines. All staff are responsible for the continual compliance with CQC standards and outcomes. The postholder must be aware of, and work in line with, the Trusts Safeguarding Adults and Children procedures.
