Summary
Act as specialist neuromuscular care coordinator, planning, delivering and reviewing care in line with best practice. Serve as a primary contact for patients, families and carers, providing information, advice and support throughout the care pathway. Coordinate care across multiple settings (secondary, community, primary care, social care and voluntary sector). Support transition from paediatric to adult services. Develop, implement and review personalised care plans, including anticipatory and symptom management. Facilitate MDT working and shared decision-making. Refer and signpost to appropriate services based on need. Promote equitable access, service consistency and pathway improvement.
