Summary

Administer saline and anticoagulants as part of Renal Replacement Therapy. Maintain vascular access whilst initiative, during and concluding renal replacement therapy. Observe for signs of infection and report to the registered nurse or appropriate medical team. Ensure relevant information is documented. Recognise the need for referral and undertake referrals to vascular access nurse and dieticians when required. Be responsible for setting specified parameters on dialysis machine, utilising mechanical infusion pump devices. Obtain requested specimens and ensure transportation to appropriate department urine, faeces, sputum, exudates e.g. swabs nose, eye, wound site etc. Co-ordinate and undertake the safe transfer of patients to and from the inpatients ward for renal replacement therapies. Provide concise handovers to other members of the nursing team. Assist in providing patients comfort during RRT treatment Provide and promote Health Education such as fluid management and renal association guidelines. Ensure the appropriate nutritional intake of all patients with due consideration to dietary and cultural needs. Utilise knowledge in the prevention of pressure sores and risk assessment tools such as Waterlow Score. Undertake the dressing of non-complex wounds (in accordance with identified competencies and in conjunction with the plan of care)
