Summary

You will work as an autonomous practitioner within theCanterbury CYPMHS multidisciplinary team, providing high-quality assessment, formulation, care planning and evidence-based intervention for children and young people up to the age of 18. You will manage an allocated caseload and work collaboratively with children, young people, families, carers and partner agencies to promote recovery, resilience, emotional wellbeing and improved functioning. Clinical responsibilities Undertake comprehensive assessments of children and young people presenting with a range of emotional, behavioural, neurodevelopmental and mental health difficulties. Complete mental state examinations, risk assessments, safeguarding assessments and safety plans. Develop clear, collaborative formulations and care plans in partnership with children, young people, parents and carers. Deliver evidence-based interventions in line with NICE guidance, CYP-IAPT principles and agreed CYPMHS pathways. Provide interventions appropriate to professional training and competence, including Cognitive Behavioural Therapy, Systemic Family Practice/Therapy, psychoeducation, parent/carer work and skills-based interventions. Monitor clinical progress using routine outcome measures and review care plans in response to changes in presentation, risk and need. Identify when specialist assessment or intervention is required and make appropriate referrals. Support timely transition, transfer or discharge planning in collaboration with relevant services. Risk and safeguarding Recognise, assess and respond appropriately to risk, including self-harm, suicidal ideation, exploitation, abuse, neglect and risk to others. Maintain clear and up-to-date risk assessments and management plans. Follow Trust safeguarding policies and local procedures, including appropriate escalation and information sharing. Participate in safeguarding meetings, professional discussions and multi-agency planning where required. Multidisciplinary and multi-agency working Contribute actively to multidisciplinary team meetings, clinical reviews, case discussions and formulation meetings. Work in partnership with schools, colleges, GPs, social care, paediatric services, early help, voluntary organisations and other relevant agencies. Communicate complex and sensitive clinical information clearly, compassionately and professionally. Promote coordinated, child-centred and family-focused care across organisational boundaries. Caseload and record management Manage and prioritise an allocated caseload, balancing clinical need, risk and service demands. Maintain accurate, timely and confidential clinical records using the Trusts electronic patient record system. Complete clinical correspondence, reports, care plans, risk documentation and outcome measures within required timescales. Ensure that all activity and clinical information are recorded in accordance with Trust standards. Leadership and service development Contribute to service development, audit, clinical governance and quality-improvement activity. Support the implementation of new policies, pathways and evidence-based practice. Share knowledge and learning with colleagues and contribute to a positive, reflective and psychologically safe team culture. Provide support, supervision or guidance to junior staff, trainees, students or support workers where appropriate. Participate in recruitment, induction and development activity when required. Professional development Maintain professional registration and practise in accordance with the relevant code of conduct. Attend regular clinical and managerial supervision. Participate in appraisal and continuing professional development. Complete mandatory and role-specific training. Work within personal competence and seek advice or supervision when presentations require more specialist input.
