Referral’s Coordinator to join our Association. The Referral’s Coordinator will be based in our offices in

Stoke.

Purpose of the role

  • As a Referral’s Coordinator, you will work as part of a team to promote our services to potential customers, stakeholders, landlords and housing partners. You will play a key role throughout the customer journey, acting as both the first point of contact for individuals seeking access to our services and a central figure in supporting their move-on into appropriate longer-term accommodation.
  • You will assess referrals to ensure that prospective residents are suitable for the service and that their support needs can be effectively met. Working collaboratively with customers and partner agencies, you will ensure individuals receive a high-quality, responsive and person-centred service that helps prevent and reduce homelessness.
  • As residents progress through their support journey, you will coordinate move-on pathways, working closely with local authorities, housing providers and landlords to secure suitable step-down or independent accommodation. Through effective referral and move-on management, you will help ensure positive housing outcomes for residents whilst supporting the efficient operation of the service.
  • You will be expected to contribute to the maintenance and development of the Association’s values, culture and ethos.
