This is an exciting opportunity to join Penn Hill Surgery as a Health & Wellbeing Coach on a 12-month fixed-term contract, jointly funded by Penn Hill Surgery and Yeovil Primary Care Network (PCN). As a Health & Wellbeing Coach, you will play an important role in supporting patients to improve their physical, mental and emotional wellbeing through personalised coaching and behaviour change techniques. Working as part of our multidisciplinary team, you will empower patients to take greater control of their health by helping them identify what matters most to them, develop realistic health goals and build the confidence and skills needed to achieve lasting improvements in their health and wellbeing. The successful candidate will work collaboratively with GPs, nurses, pharmacists and other healthcare professionals to provide holistic, person-centred care. The role focuses on improving patient outcomes, reducing health inequalities and promoting self-management, while ensuring patients are connected to the most appropriate services and community resources. The role will include: Delivering one-to-one and group health coaching sessions using a personalised, strengths-based coaching approach. Supporting patients to identify what matters most to them, develop achievable goals and create personalised action plans. Encouraging sustainable behaviour change and supporting patients to overcome barriers affecting their health and wellbeing. Managing and prioritising your own caseload, ensuring patients receive timely support and appropriate follow-up. Working collaboratively with the multidisciplinary team to ensure patients receive coordinated, joined-up care. Signposting patients to appropriate community groups, voluntary organisations and local support services. Maintaining accurate patient records using EMIS Web and recording interventions using the appropriate clinical coding. Collecting outcome data and contributing to service evaluation, quality improvement and the ongoing development of personalised care services. Promoting personalised care and supported self-management throughout the practice and wider Primary Care Network. We are looking for someone who is compassionate, organised and motivated, with excellent communication and interpersonal skills. You will enjoy building trusting relationships with patients and colleagues and be committed to delivering high-quality, person-centred care. Experience of supporting behaviour change, coaching or working with people in a health, care or community setting would be advantageous, although applicants with transferable skills and a passion for helping others are encouraged to apply. As a valued member of our team, you will receive regular supervision, annual appraisal and opportunities for continuing professional development. You will work within a supportive multidisciplinary environment where collaboration, learning and continuous improvement are encouraged. The successful applicant will be expected to work in accordance with Penn Hill Surgery's policies and procedures, maintaining high standards of confidentiality, safeguarding, equality and patient care at all times. This post is subject to an Enhanced Disclosure and Barring Service (DBS) check. If you are passionate about helping people improve their health and wellbeing and would like to be part of a friendly, supportive practice committed to delivering personalised care, we would love to hear from you.
