Summary
Deliver chronic disease management clinics (e.g. diabetes, asthma, COPD and hypertension). Undertake cervical screening in line with the NHS Cervical Screening Programme. Administer childhood and adult immunisations and vaccinations under PGDs/PSDs. Provide wound care, dressings, venepuncture, NHS Health Checks, and other agreed clinical procedures. Promote health and wellbeing through patient education and preventative care. Maintain accurate patient records using EMIS Web and ensure high standards of clinical documentation. Support the achievement of Quality and Outcomes Framework (QOF) and other contractual requirements. Participate in clinical audit, quality improvement and infection prevention and control activities. Work collaboratively with GPs and the wider multidisciplinary team, referring patients appropriately and contributing to the delivery of high-quality patient care. Maintain professional competence through continuing professional development and mandatory training, practising in accordance with the NMC Code, NICE guidance and CQC standards. providing advice about contraception providing emergency first aid and treatment giving advice, education and information about health conditions and ailments, stopping smoking and losing weight taking patient samples, swabs and specimens, and checking pulses, temperatures and blood pressures writing records and keeping patients notes up to date Running and completing clinical audits
