Summary
Join our dedicated Learning Disability Care Hub and help improve the health, wellbeing and life opportunities of people with learning disabilities across our communities.
As a Band 5 Community Learning Disability Nurse, you will work as part of a supportive multidisciplinary team, delivering specialist pathways, evidence-based interventions and person-centred care to individuals with learning disabilities and complex needs
Managing your own caseload within the community.
Undertaking specialist health and behavioural pathways, assessments.
Supporting individuals in a range of settings, including their own homes.
Promoting health, wellbeing and reducing health inequalities for people with learning disabilities.
Working closely with families, carers, primary care, social care and partner agencies.
Contributing to specialist pathways including Positive Behaviour Support, Physical Health, Autism, Epilepsy, Dysphagia and Dementia.
