Summary
Complete specialist, complex assessments demonstrating specialist knowledge of the concepts of biological, physical, social, and psychological sciences in relation to function to ensuring patient safety in their own homes Complete comprehensive holistic assessments to address immediate presenting needs of the patient utilising extended clinical skills Complex manual handling including urgent prescription of specialist equipment to manage immediate risks to patients during a crisis situation Profession-specific assessments, generalist assessments and, where appropriate and within scope of extended practice, cross-profession assessments to maintain patient safety during acute medical / social crisis Refer to appropriate clinical services to ensure patient safety is maintained; escalation of patient deterioration Use assessment tools/skills that will ensure an appropriate level of recovery intervention so that patients who present with highly complex needs are timely referred to the appropriate clinicians/teams Complete complex risk assessments, both as an independent clinician and drawing on the wider MDT as appropriate Triage and prioritisation of new referrals and allocation of workload To be responsible for the day-to-day management and prioritisation of your own caseload Respond to urgent referrals and an unpredictable workload: this will require prolonged concentration in an unpredictable environmen Be wholly accountable for your clinical practice taking every reasonable opportunity to sustain and improve knowledge and professional competence To manage a varied caseload, seeking support from peers and senior staff where necessary to ensure the provision of a timely, clinically effective service based on evidence based practice To be actively involved in service development projects towards the development and delivery of UCR, therapy and broader community services, and to contribute to new developments and initiatives within the locality Play an active role in the management of clinical governance and patient safety For more information please see attached job description and person specification
