Summary
Ensure patient involvement is integral to all service improvement activity.Develop strong links with patient and carer groups identifying opportunities for involvement and engagement in any service redesign activity.Ensure appropriate referral and care pathways across service areas are cohesive and interface effectively to ensure smooth transition of patients.Work collaboratively with the service Clinical Leads thus ensuring that operational process is aligned to professional practice.Ensure staff keep accurate and comprehensive patient records (written and electronic) in line with professional and Trust standards of practise, adhering to guidance and best practice If successful this vacancy will require a Standard DBS check at a cost of £26.90. This cost will be deducted from your first Trust salary In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
