Summary
Clinical responsibilities: Undertake structured medication reviews for patients with complex medication regimens. Optimise medicines for patients with long-term conditions. Conduct medication reviews following hospital discharge. Manage medicines reconciliation following discharge from hospital or outpatient appointments. Provide prescribing advice to GPs and other clinicians. Support safe prescribing for patients receiving high-risk medicines. Undertake medication monitoring in accordance with national and local guidance. Review repeat prescribing requests. Respond to medicines-related patient queries. Manage prescribing for care home residents where appropriate. Participate in multidisciplinary team meetings. Provide patient education regarding medicines and treatment options. Support deprescribing where clinically appropriate. Monitor prescribing safety alerts and implement necessary actions. Management of patients with long term conditions Medicines Optimisation Promote evidence-based prescribing. Undertake prescribing audits. Support QOF achievement. Support Medicines Optimisation initiatives. Improve prescribing quality indicators. Reduce medicines waste. Monitor prescribing budgets where appropriate. Implement MHRA safety alerts. Support implementation of NICE guidance.Quality Improvement Participate in clinical audit. Lead quality improvement projects. Analyse prescribing data. Identify opportunities to improve patient care. Support achievement of national and local contractual requirements. Assist with CQC preparation. Patient Safety Identify prescribing risks. Investigate medicines-related incidents. Participate in Significant Event Reviews. Promote safe prescribing systems. Support antimicrobial stewardship. Teaching and Training The Clinical Pharmacist will: Support trainee Clinical Pharmacists. Support Community Pharmacist placements. Contribute to multidisciplinary education. Deliver medicines education sessions to practice staff. Participate in practice educational meetings. Maintain their own continuing professional development.
