Summary

To collect specific information to support theassessment, treatment and management of communicationand swallowing needsfor service usersreferred, for example,use telephone screening tools; gather information for the Pre assessment Communication Profile from carers/ staff/ family members, as requested by SLT. Toapplyskillsand knowledgeinAlternative andAugmentativeCommunication(AAC)including manual signing,symbolandcommunication aiduseacross a variety of communication environments. Tocontribute todevelopinginformation that is accessible to service users, staff and carersasrequired. Toprepare requestedcommunicationresourcesrequired by the SLT team,thesemay be to support specific work with services users and/ or to supporta range oftrainingpackagesdelivered by SLT.This will involve use of symbols software. To discuss and agree withrelevant TeamSLTany otherrequests forMulti disciplinaryteammemberswork. To use observation skills tonoteservice user and carer/supportstaff communication (including verbal, non-verbal communication). To seek advice from supervising SLT if a service users statusindicatesadjustment to a treatment plan. To develop skillstoprioritise and manageown workload, requesting discussion and support when needed. To contribute informationabout own workwithspecific service userstowiderdiscussion aboutthoseindividuals,e.g.at integrated team meetings/case discussions, etc. To work collaboratively with service users, theircarersand families asappropriate, to make decisions relevant to the serviceusersindividualcommunicationprogrammes in conjunction with relevantTeamSLT. Toattend and contribute actively tomonthlySLT andotherrelevant LDP meetings. To implement and reviewSLTcareplans forservice usersand informTeamSLT of current situation. Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.
