Summary

The Care Coordinator Team is primarily responsible for supporting the Canterbury PCNs in the collaborative delivery of services and supporting the care for cohorts of patients. The main focus of the role will be on coordinating the care of identified cohorts of patients and ensuring PCN services are delivered effectively. The Care Coordinator might be required to liaise with patients and, if appropriate, their carers, before or after the patients consultation with a clinician or other healthcare professional. You might also be given a caseload of identified patients and be required to ensure that their changing needs are addressed by taking into account local priorities, health inequalities and/or population health management risk stratification. The role includes: Working closely with PCN teams and the wider Care Coordinator Team Liaising with health and care professionals, practice teams, patients and carer supporting collaborative working across Canterbury Supporting the delivery of PCN services Providing a direct link between care homes, practices and PCN teams Helping to establish and maintain effective systems and processes Send feedback questionnaires and batch messages using IT platforms The role will evolve and develop in line with emerging needs and priorities of services and the local system. The role will be based mainly at our premises on Ethelbert Road. It will be necessary to have the ability to travel around Canterbury and the surrounding area, between practices and to be able to work remotely some of the time.
