Summary

We are looking for a dedicatedSupport Workerto join our team and play a vital role within our multidisciplinary care team. In this position, you will work alongside service users to assess their needs, co-create personalized care plans, and deliver evidence-based interventions using theDialog+framework.

Working across boundaries withinMental Health TogetherorMental Health Together Plus, you will collaborate closely with Clinical Pathway Leads, facilitate group-based sessions, and help individuals connect with community and leisure activities. Using compassionate listening, basic CBT formulation models, and mindfulness approaches, you will support service users in making sense of their mental health and developing practical coping strategies.

Above all, you will be a champion for quality care—promoting dignity, equality, patient safety, and mental well-being for our service users and their carers at every step.

• Assessment & Personalised Planning:Contribute to assessing clients referred toMental Health TogetherorMental Health Together Plus. Work alongside service users to identify their needs and develop ongoing care plans.

• Delivering Interventions:Use the Dialog+ framework to guide care, deliver group-based interventions, and apply basic CBT formulation and mindfulness techniques to help service users build effective coping strategies.

• Community Engagement:Support individuals to access and actively participate in local community networks, social groups, and leisure activities.

• Active Listening & Carer Support:Listen empathetically to help service users understand their mental health challenges, while offering high-quality support to their families and carers.

• Collaborative Care:Work alongside Clinical Pathway Leads and multidisciplinary team members across organizational boundaries to ensure joined-up care delivery.

• Quality & Innovation:Actively influence and drive positive change, promoting best practices, patient safety, equality, and an excellent care experience at all times.
