Job Description – Support Worker

Job Title: Support Worker

Reports To: Registered Manager / Care Coordinator

Location: Middlesbrough

Hours: As required

Job Purpose

To provide high-quality, person-centred care and support to individuals in their own homes and the community, promoting independence, dignity, choice, and wellbeing while working in accordance with company policies and CQC Fundamental Standards.

Key Responsibilities

Deliver person-centred care in line with individual care plans and risk assessments.

Support with personal care, including washing, dressing, toileting, grooming, and mobility where required.

Assist with meal preparation, nutrition, hydration, and domestic tasks such as cleaning, laundry, and shopping.

Administer or assist with medication in accordance with training and company procedures.

Encourage independence and support individuals to access social, leisure, and community activities.

Provide emotional support, companionship, and reassurance.

Observe, monitor, and promptly report any changes in a person's health or wellbeing.

Maintain accurate and timely records, including care notes and Medication Administration Records (MAR).

Follow all safeguarding, health and safety, infection prevention, confidentiality, and company policies.

Work professionally with colleagues, families, and other healthcare professionals.

Attend mandatory training, supervision, team meetings, and continuous professional development.

Work flexibly, including evenings, weekends, and bank holidays where required.

General Responsibilities

Promote dignity, privacy, equality, diversity, and inclusion.

Maintain professional boundaries and act with honesty and integrity.

Report safeguarding, health and safety, or other concerns immediately.

Comply with all relevant legislation, including the Health and Safety at Work Act, Mental Capacity Act, GDPR, and CQC regulations.

Undertake any other reasonable duties requested by the Registered Manager.
