Summary
Clinics When attending clinics, please arrive promptly and preferably early to review notes and enable the smooth running of the clinic. You may be asked to see a mixture of new and review patients. Patients notes are of great importance and are a legal document. Any entries must be dated, legible and have your signature, printed name and bleep number at the end of the entry. A clinic letter should be complete for each patient you see, training on the Trusts digital letter writing will be provided by a secretary. It is your responsibility to ensure that any letters which you write are signed electronically as soon as possible so as to be posted out in a timely manner. This applies even if you are on leave you can ask someone else to sign letters for you, but it is responsibility to ensure this happens. Letters need to be clear and concise and have all the relevant information present as well as the relevant CCs. Theatres Clerk each patient promptly on arrival (most will have been pre-assessed, however ensure this has been done). Check carefully that the results of all investigations arranged in Outpatients are available and written in the notes. Check radiographs and scans are available and up to date. Make sure the consent form is completed and signed. You should not consent patients for operations you are not familiar with. Check that all procedures are included and discuss with the consultant if in doubt. If for extractions, make sure all teeth are accurately charted. Record deciduous teeth as CAPITAL letters please, ie ABCDE. Check teeth for removal are correctly documented. When documenting the operation, write the details, including the side of the procedure IN FULL, to avoid confusion. Patients having an operation on one side of their body need to be marked with indelible ink and told not to wash this off. Liaise with the Anaesthetists for the list if there are any concerns regarding the patient. If the anaesthetist asks for additional investigations, ensure these are done in a timely manner. Prescribe any drugs required as part of procedure including prophylactic antibiotics, steroids, VTE prophylaxis (as well as documenting VTE assessment), analgesics and regular medications. You will be responsible for the teaching of junior colleagues and students in theatre so use these opportunities to develop your portfolio. On-call Duties When on-call you can expect telephone calls from GPs, GDPs, patients and spoke hospitals, as well as from within our hospital, normally referrals are taken by the first on call. During the day time there will be one junior to cover the wards and one to cover emergency referrals, but after 5pm there is only one junior doctor to cover both areas. The night junior (after 8pm) also cross covers the Plastic on call rota. At weekends the Registrar rota is supplemented by locums (usually experienced juniors currently studying for their second degree). Please be aware of all team members and their experience. Maxillofacial Surgery on-call rota is shared by the Consultants on a 1:9 on-call rota. During their on-call all routine activity is reduced if possible and a Consultant is usually available to supervise all Emergency and semi-elective surgery. If you are unable to come to work or are sick, Human Resources should be notified, the Clinical lead as well as the on-call Consultant and Goodshape absence management system. An absence form needs to be completed on your return to work.
