Summary
The role will be integrated within the existing Community Childrens Nursing Team and the Childrens Continuing Care Team, who are part of an integrated network of services for children and young people in Bexley and Greenwich providing high quality specialist child centred care. The team of specialist and generic childrens community nurses provides skilled, high quality, comprehensive nursing care to children and families with nursing needs in their own homes or community settings. The post holder will be responsible for contributing towards ongoing developments of clinical practice, research and standards of care within the service, including policies, protocols, guidelines and collaborate with the wider multidisciplinary team implementing acute pathways between the hospitals, GP and diagnostic hubs into the community. The post holder will provide a high standard of evidenced based nursing care for children and young people with the following conditions: children with disabilities and complex conditions including those requiring palliative and end of life care. To provide clear visible leadershipsupervision, caseload management and professional clinical expertise to the clinical skills nurse trainers in the team, to ensure every aspect of the child and families experience is delivered in a safe, efficient and evidence-based manner.
