Summary

To be involved and lead in the delivery of high standards of health care to any patient registered with a Norfolk or Waveney GP, by coordinating and delivering assessments, planning and implementing and evaluating health care for complex patients with Post Covid Syndrome/Long Covid. To lead on developing practice, procedures / systems for the team and provide clinical support within own area of expertise including clinical assessment delivery for highly complex patients or when required during times of high pressure within the service. To develop and sustain working relationships with system partners delivering the Post-COVID Service processes and patient focussed planning. (eg. CCG/ICB/NSFT/Active Norfolk.
