About the team

The Hertfordshire Home Improvement Agency (HHIA) supports residents, living with a disability to remain safe, independent, and well within their homes through delivery of Disabled Facilities Grants (DFGs) and housing adaptations.The service works collaboratively with Adult Social Care, district councils, housing associations, and health partners to reduce hospital admissions, facilitate timely discharges, and improve quality of life for residents. The HHIA 6 District partnership consists of the following:

  • Watford
  • Broxbourne
  • Stevenage
  • North Hertfordshire
  • East Hertfordshire
  • St Albans

About the role

As a Case Worker within Hertfordshire Home Improvement Agency (HHIA), you will support older and disabled residents to access grants, adaptations and practical assistance that enable them to remain safe and independent in their own homes.

You will manage a varied caseload from initial referral through to completion, conducting home visits and customer reviews to assess needs, gather information and explain available services and funding options.

You will undertake financial assessments and means tests, ensuring that customers receive accurate advice and support regarding grant eligibility and contributions.

You will coordinate Disabled Facilities Grant (DFG) applications and associated housing assistance, liaising with customers, Occupational Therapists, contractors, district councils and other partner organisations to progress cases efficiently and achieve positive outcomes.

You will act as a key point of contact for customers, providing clear advice, guidance and reassurance throughout their adaptation journey, ensuring a high standard of customer care at all times.

You will maintain accurate records and case management systems, monitoring progress against timescales and ensuring that all work complies with relevant legislation, policies and procedures.

You will work independently across Hertfordshire, planning and prioritising your workload to manage competing demands whilst meeting service targets and deadlines.

Through your work, you will make a direct impact on residents' quality of life by helping them access adaptations and support that improve safety, accessibility and independence within their homes.

About you

Essential:

  • Experience of working in a customer-focused, housing, health, social care or support environment.
  • Experience of managing and prioritising a varied workload or caseload.
  • Strong communication and interpersonal skills.
  • Ability to work independently and use initiative.
  • Good organisational and administrative skills with attention to detail.
  • Experience of maintaining accurate records and using IT systems, including Microsoft Office.
  • Commitment to delivering excellent customer service.
  • Full UK driving licence.
  • Access to a vehicle for business use and willingness to travel throughout Hertfordshire.

 Desirable

  • Experience of working with older people and/or people with disabilities.
  • Knowledge of Disabled Facilities Grants (DFGs) or housing adaptation services.
  • Experience of undertaking financial assessments or means testing.
  • Knowledge of housing, health or adult social care services.
  • Experience of conducting home visits and working with partner organisations.
  • Relevant qualification in housing, health, social care or a related field.
