Summary

An exciting opportunity has arisen for a Senior Mental Health Practitioner to join a Primary Care Network (PCN) within the Neighbourhood Mental Health Service (NMHS) in Stockport. This role supports the transformation of community mental health services through integrated, person-centred care, strengthening links between primary care, secondary mental health services, physical health services, and community partners.

Working within the service, the post holder will provide mental health leadership, support pathway development, promote collaborative working, and improve access to timely mental health support. Employed by Pennine Care and aligned to a designated PCN, you will work closely with GPs, primary care teams, service users, and neighbourhood partners to improve integration, continuity of care, and access to specialist support, helping to deliver coordinated, recovery-focused care that improves outcomes and reduces barriers between services.

The service supports adults aged 18 and over whose needs have not improved through primary care interventions, who are unsuitable for NHS Talking Therapies, or whose needs exceed service thresholds without meeting secondary care criteria. It also supports people with serious mental illness (SMI) who are stable and can be supported through a neighbourhood-based approach, as well as people with SMI requiring physical health checks, medication reviews, and enhanced access to physical health interventions.

You will

• Provide a combined consultation, advice, triage and liaison function, informed by NICE guidance within the primary care setting

• Support an access function for people, ensuring swift and seamless access to the most appropriate care pathway and onward access to mental health services

• Develop and implement mental health clinics based on the needs of the PCN

• Directly support GP’s with the management of more complex mental health presentations and linking with secondary care mental health services as appropriate

• Support people presenting in crisis, assessing clinical risk, and providing short term interventions, where possible, to manage the crisis, link into mental health support networks and promote self-management

• Provide a link role between neighbourhood primary care services and secondary care mental health services for complex cases and issues

• Identify training and education needs for PCN teams in relation to mental health practice and support the development of mental health training as required.
