The Wheelchair Service currently operates 5 days per week Monday to Friday. The post is 37.5 hrs per week working 8.30am to 4.30pm each day. Why you should join our Team You must be able to demonstrate a good level of communication, interpersonal skills, team working, motivation, initiative and enthusiasm which are essential in establishing working relationships within the MDT. The successful candidate will work as part of the Leeds Wheelchair Services Multi- disciplinary team based at Seacroft Hospital as part of the Specialist Rehabilitation Services Department. What you will bring to the role As the successful candidate you will play a key role in the day to day running of the department, accepting and triaging patients referred to Leeds Wheelchair Service. The role involves skilled administrative duties, face to face assessments in clinic and the community, as well as supporting the wider Multidisciplinary Team from health and social care colleagues across the Leeds ICB area. As a Therapy Support Worker, you will be responsible for designated patients, planning and prioritising the caseload/referral appropriately, involved in the assessment and provision of Wheelchairs and the associated equipment/accessories. Patients will be assessed according to set protocols, care pathways and guidelines. The majority of the caseload is carried out via telephone assessments, good IT and organisational skills are required. It is essential to have relevant experience in this area and/or be working towards a NVQ level 3 in health or social care related subject. A driving license/ability to travel across Leeds is also essential; this includes travel between Trust premises and patient homes for community visits The individual must demonstrate the Leeds Way Values.
