Summary

Assess service users referred with memory problems using the agreed assessment protocol and tools. All interventions (assessment and post diagnosis) to consider any specific physical, cultural, psychological, spiritual and age-related needs. Work collaboratively with other service providers health, social/community and voluntary, including using social care procedures to directly request community services when appropriate. Contribute to the delivery of non-pharmacological post diagnostic interventions. Maintain own personal and professional status by ensuring the requirements laid down by the relevant professional body (HCPC/NMC) e.g., registration and continuity of professional development. Have an informed knowledge and understanding of the Mental Capacity Act 2005, including the Deprivation of Liberty Safeguards, and its application to the care and treatment of people with dementia and cognitive impairment, and therefore be able to offer appropriate advice to service users and their carers.
