Job DescriptionJob Title Practice Nurse Reports To Practice Manager / GP Partners Location Stafford Health and Wellbeing, Whitgreave Court, Stafford, ST16 3GS Hours 25 per week Salary Dependent on experience and qualifications Job Purpose The Practice Nurse is responsible for delivering high-quality, evidence-based nursing care to patients within a General Practice setting. The post holder will work collaboratively with GPs, healthcare professionals, administrative staff, and external agencies to promote health, prevent disease, manage long-term conditions, and provide patient-centred care. The Practice Nurse will contribute to the achievement of Quality and Outcomes Framework (QOF) targets, national and local healthcare priorities, and the overall delivery of safe and effective healthcare services. Key ResponsibilitiesClinical Duties The Practice Nurse will: Provide safe, effective nursing care within their scope of professional practice. Undertake comprehensive patient assessments and develop appropriate care plans. Deliver health promotion and disease prevention interventions. Administer vaccinations and immunisations in accordance with national and local guidelines. Undertake cervical screening procedures. Perform wound care management, including dressings and ulcer care. Manage patients with long-term conditions such as: Diabetes Asthma COPD Hypertension Cardiovascular disease Conduct NHS Health Checks and lifestyle assessments. Carry out ECGs, blood pressure monitoring and other diagnostic procedures as required. Take and process specimens including venepuncture where trained and competent. Administer prescribed medications and treatments under Patient Group Directions (PGDs), or other authorised frameworks. Provide travel health advice and vaccinations where applicable. Support patients with smoking cessation, weight management, alcohol awareness and healthy lifestyle choices. Recognise and respond appropriately to safeguarding concerns involving children and vulnerable adults. Participate in infection prevention and control activities. Patient Care The Practice Nurse will: Ensure patients are treated with dignity, respect and compassion. Promote patient involvement in decision-making regarding their care. Maintain confidentiality in accordance with NHS and professional standards. Provide education, advice and support to patients, relatives and carers. Advocate for patients and ensure equitable access to services. Clinical Governance and Quality The Practice Nurse will: Maintain accurate, timely and comprehensive clinical records. Participate in clinical audits and quality improvement initiatives. Support the achievement of QOF and enhanced service requirements. Report and manage incidents, risks and near misses appropriately. Adhere to infection prevention and control policies. Work within all practice policies, procedures and professional standards. Contribute to service development and improvement initiatives. Team Working The Practice Nurse will: Work collaboratively with GPs, Nursing Associates, Healthcare Assistants, PCN staff and the wider multidisciplinary team. Support and mentor less experienced staff where appropriate. Participate in practice meetings and clinical discussions. Develop effective working relationships with external healthcare providers and community services. Professional Responsibilities The Practice Nurse will: Maintain current NMC registration and comply with the NMC Code. Undertake mandatory and professional training required for the role. Participate in annual appraisal and personal development reviews. Keep knowledge and skills up to date through continuing professional development (CPD). Work within professional competency and seek support when required. Demonstrate a commitment to equality, diversity and inclusion. Person SpecificationEssential Qualifications Registered Nurse (Adult) with current NMC registration. Evidence of continuing professional development. Essential Experience Experience working within a healthcare environment. Experience delivering patient care independently. Knowledge of chronic disease management. Experience of health promotion and preventative healthcare. Desirable Experience Experience working in General Practice or Primary Care. Cervical Cytology qualification. Immunisation and Vaccination training. Chronic Disease Management qualification. Independent Prescribing qualification. Essential Skills Excellent communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Strong organisational and time management skills. Competence in clinical assessment and decision-making.
