Summary
Key Clinical Responsibilities Undertake face-to-face, telephone, online and, where agreed, home visit consultations in accordance with the practice timetable and clinical need. Assess, diagnose and manage patients presenting with undifferentiated and undiagnosed conditions, making appropriate professional and autonomous clinical decisions. Develop, review and implement care plans in consultation with patients and, where appropriate, carers and other healthcare professionals. Prescribe safely, effectively and appropriately in line with current legislation, professional guidance, local formularies and practice prescribing policies. Issue, review and sign acute and repeat prescriptions in accordance with practice protocols and clinical governance requirements. Make appropriate referrals to secondary care, community services and other healthcare providers, ensuring relevant clinical information is communicated accurately and promptly. Maintain clear, accurate, contemporaneous and complete clinical records in accordance with GMC guidance, practice policies and applicable data protection requirements. Provide health promotion, disease prevention, screening, counselling and patient education as appropriate. Participate in audit, quality improvement and clinical standard setting to support safe and effective patient care. Undertake all reasonable clinical duties and responsibilities normally associated with a General Practitioner working within NHS primary care. Take part in extended hours work along with other GPs and other health care professionals in the practice/PCN. 3. Professional, Legal and Regulatory Responsibilities Maintain full registration with the General Medical Council, inclusion on the appropriate performers list, and any other professional requirements necessary to practise as a GP in the United Kingdom. Practise in accordance with GMC standards, including Good Medical Practice, and comply with all applicable legislation, NHS requirements, contractual obligations and practice policies. Maintain appropriate medical indemnity or professional protection arrangements as required for the role. Work within the limits of professional competence, seeking advice and escalating concerns where appropriate. Comply with requirements relating to revalidation, appraisal, continuing professional development and mandatory training. Act at all times with integrity, professionalism, respect and accountability, maintaining public confidence in the profession and the practice. 4. Confidentiality and Information Governance The post-holder will have access to confidential information relating to patients, carers, staff, healthcare professionals and the business of the practice. All such information must be treated as strictly confidential and handled in accordance with practice policies, the UK General Data Protection Regulation, the Data Protection Act 2018, the common law duty of confidentiality, NHS information governance requirements and any other applicable legal or regulatory obligations. Confidential information must only be accessed, used, disclosed or shared where there is a lawful basis and a legitimate professional need to do so. The post-holder must report any actual or suspected data breach, confidentiality incident or information governance concern promptly in accordance with practice procedures. 5. Safeguarding Safeguard and promote the welfare of children, young people and adults at risk. Comply with all practice safeguarding policies and local safeguarding procedures. Maintain safeguarding training at the level appropriate to the role. Recognise, document and escalate safeguarding concerns promptly and appropriately. Work collaboratively with safeguarding leads, statutory agencies and other professionals where required. 6. Health, Safety and Infection Prevention Comply with the practice health and safety policy, infection prevention and control policy, and all relevant procedures. Use personal security systems, protective equipment and safe working practices in accordance with practice guidance. Identify, report and, where appropriate, act upon hazards, risks, incidents, near misses and infection prevention concerns. Follow safe systems of work for specimens, sharps, clinical waste, medicines, equipment and premises. Maintain a clean, safe and professional working environment for patients, staff and visitors. Complete mandatory training and contribute to a culture of safety and continuous improvement. 7. Equality, Diversity and Inclusion The post-holder must support equality, diversity, inclusion and human rights in all aspects of their work. They must treat patients, carers, colleagues and visitors with dignity, courtesy and respect, and act in accordance with current equality legislation, professional standards and practice policies. The post-holder must not discriminate, harass, victimise or otherwise treat individuals unfairly on any unlawful basis. 8. Communication and Team Working Communicate clearly, respectfully and effectively with patients, carers, colleagues and external agencies. Recognise and respond appropriately to patients communication needs, including alternative methods of communication where required. Work collaboratively with clinical, administrative and multidisciplinary colleagues to support safe, coordinated and efficient care. Contribute constructively to practice meetings, service development, clinical governance activity and team learning. Escalate clinical, operational or professional concerns promptly through the appropriate practice channels. 9. Quality, Audit and Continuous Improvement Contribute to maintaining and improving the quality, safety and effectiveness of services provided by the practice. Participate in audit, significant event analysis, learning reviews and quality improvement activity where appropriate. Identify and report risks, incidents and areas for improvement in accordance with practice procedures. Manage time, workload and resources effectively and professionally. Support evidence-based practice and the implementation of agreed clinical standards, policies and guidance. 10. Training and Professional Development Maintain professional knowledge and skills through continuing professional development, appraisal and revalidation. Complete all mandatory and statutory training required by the practice and relevant regulatory bodies. Take responsibility for personal learning, performance and professional development. Participate in annual review processes and maintain appropriate records of learning and development. Share knowledge and support the learning of colleagues where appropriate. 11.
