If so, we have a brilliant opportunity to come and join us here in the District Therapy Team at Bolton Foundation Trust. We work across six neighbourhoods in Bolton to help provide a holistic community-centred provision.

We are seeking to appoint a dynamic, confident, innovative and flexible individual with excellent communication and organisational skills. The successful individual will need to engage closely with acute staff, community colleagues and social care staff to coordinate and provide high quality care in the community to avoid hospital admission and support hospital discharges.

Service is over 7 days 8am-6pm service. Staff currently work 1 in 6 weekends.

In order to successfully undertake this post you must be an experienced Physiotherapist looking to develop in this clinical area.

You will need some knowledge of community/intermediate care services.

The role will include participating in effective multi-disciplinary team meetings and integrated working with the Local Authority.

If you are looking for a opportunity to develop at Band 6 level  or wish to strengthen existing skills in a new clinical area that incorporates direct patient care, teaching, leadership and service development and possess all the above qualities we would be delighted to hear from you.
