An exciting opportunity has come up for the Tissue Viability Service. We are currently looking for a Band 5 Tissue Viability Community Nurse to work across Hounslow based at Heart of Hounslow Centre for Health, Bath Road. The service operates from 0900 – 1700 Monday to Friday.

Collaboratively working with our District Nurses, primary care, integrated rapid response teams, community matrons and social service colleagues are essential to ensure seamless care to our patient group.

To provide high quality, skilled nursing care in Tissue Viability and to assist the Tissue Viability Nurses in the on-going planning, implementation, and evaluation of the holistic needs of the patient/client, family, and/or carer.

 

To lead and manage the Leg Ulcer clinic at multiple sites with the support from the TV lead nurses.

 

To work jointly with the Practice Nurses, community nurses, Pressure Ulcer Prevention Nurse, and other healthcare providers in relation to wound care.

 

Offers/performs clinic appointment and home visits.

 

1.      To assist the Tissue Viability Nurses in coordinating the work of the service, to ensure effective caseload and care management.

2.      To effectively and independently manage the Tissue Viability/Leg Ulcer Clinics across Hounslow with support from Tissue Viability Nurses.

3.      To assist in coordinating and supervising the work of the TV team, for a span of duty in their absence.

4.      To undertake first assessments and reviews, planning, implementation, monitoring, and evaluation of individual programmes of care once deemed competent.

5.      To deliver a high-quality service to clients and carers independently planning, implementing and evaluating programmes of care

6.      To have knowledge across the range of work procedures and practice underpinned by theoretical knowledge or relevant practical experience e.g. wound care, leg ulcer management and Doppler assessment.
