Summary

Are you ready to join our established multi-disciplinary specialist team at Hampshire Hospitals NHS Foundation Trust

We are seeking a motivated and enthusiastic individual to join our Inflammatory Bowel CNS team at Basingstoke and North Hampshire Hospital.

The successful candidate will ideally have previous experience in Gastroenterology and an interest in Inflammatory Bowel Disease. Although based in Royal Hampshire County Hospital there may be requirement to support patients within our Basingstoke and North Hampshire Hospital service.

Who we are:We have a well-established Gastroenterology service at each of our main hospital sites, Basingstoke and North Hampshire Hospital and Royal Hampshire County Hospital, with three members of the IBD CNS team based at each site.

We are looking for the successful candidate to join our established team to support in the delivery of care for our Inflammatory Bowel Disease patients.

 

• Use clinical skills to assess patients with IBD referred from the wards and departments within the trust and Primary Care.

• To be an expert resource in IBD to healthcare professionals, people with IBD and their family/carers

• To work in collaboration with all members of the multidisciplinary healthcare team to promote excellent standards of care to people with IBD in line with evidence based practice

• To provide education to people with IBD and/or their carer’s, and to professionals who care for people with IBD

• To practice clinically in a way that supports & facilitates patients self-management

• To work in collaboration with other health care professionals to develop new and innovative tools to improve patient care

• To work in collaboration with patients and their family/carers to develop individualised care plans providing knowledge and support enabling them to self manage their IBD
