To strategically lead, develop and implement a high quality clinical pharmacy service as part of the multidisciplinary teams in the Planned and Critical division

To lead on developing a strategic vision and roadmap for pharmacy services within the division, taking into account the diverse skills of the wider pharmacy team

To work with the multidisciplinary team to promote safe, effective use of drugs in the specialist clinical area, evaluating available information and providing guidance to the relevant directorate/division on new drugs.

To provide an advanced clinical service to patients as part of the team. This may include patient facing work in areas such as: Surgery, Critical Care, Trauma, MSK or parenteral nutrition

To lead and manage the pharmacy clinical team working in the Planned Care division and ensure service delivery to agreed standards in line with business planning requirements.

To ensure the development and delivery of training in the specialist clinical area for pharmacy staff and other staff groups as required.

To support audit and research within teams and promote the use of Quality Improvement (QI) methodology at all opportunities

1. Strategically lead, develop and implement a high quality clinical pharmacy service as part of the multidisciplinary teams within planned care

2. Work closely with the division Clinical Director and Director of Operations to align clinical pharmacy services with wider Trust patient and business needs

3. Act as the interface between Clinical Service Managers within the division and Pharmacy teams

4. Act as a leader, expert clinical practitioner and role model within pharmacy for the specialist clinical area.

5. Providing support and advice to other health care professionals through participation in relevant professional groups and appropriate working groups, attendance at conferences and educational events and practice development.

6. Demonstrate an advanced level of professional reasoning, judgement and accountability and manage difficult and ambiguous problems, making decisions in situations where there is limited information.

7. Demonstrate expert practice in the planning and delivery of pharmaceutical care for patients in the specialist clinical area across the Trust. 8. Liaise with the senior pharmacy management team, Clinical Director for the specialist clinical area, senior clinical nurses, consultants, other members of the multidisciplinary team and relevant Trust committees to ensure that treatment plans, policies, procedures and guidelines are appropriately agreed, implemented, monitored, evaluated and reviewed in order to optimise patient benefit.

9. Apply advanced level clinical reasoning and decision making to ensure that national guidance and research evidence is integrated into medicines optimisation for the relevant patient population.

10. Liaise with the Principal pharmacist Formulary and Medicines Management in updating relevant sections of formulary and lead on the introduction of new medicines into clinical practice.

11. Contribute to any relevant multidisciplinary policies within the speciality and support the work of the Trust wide Medicines Management and Clinical Guidelines Committee.

12. Implementation of relevant local and national guidelines within specialist areas

13. Report incidents using the appropriate Trust systems and contribute to the identification and lead the investigation of drug-related incidents, medication errors, patient concerns and complaints and serious incidents within the specialist clinical area; liaising with the Principal Pharmacist for Medication Safety and the Deputy Chief Pharmacist(s) as appropriate, providing feedback and identifying learning for the individual, pharmacy, specialist area or across the Trust.

14. Lead the relevant pharmacy team to ensure audits service evaluations and quality improvements are undertaken and results disseminated appropriately, liaising with the Deputy Chief Pharmacist, Clinical Services regarding quality improvement that may be identified.

15. Practice as a non-medical independent prescriber within Trust guidelines or working towards qualification if not yet achieved.

16. Be available and receptive to inquiries from other pharmacy team members and MDT and about individual patients within the in the specialist clinical area(s), maintaining appropriate patient confidentiality.

17. Liaise with appropriate members of the pharmacy and multidisciplinary teams to identify and implement solutions when supply issues arise for necessary medication.

18. To act as the link pharmacist to the relevant clinical networks across SWL and Surrey Heartland system

19. To liaise and work with other specialist pharmacists (both within SWL and nationally) to share the best practice to strengthen patient-focused pharmaceutical care.

20. Support the Deputy Chief Pharmacist(s) with the implementation of novel ways of working, utilising digital tools to help optimise patient care.

21. Develop the pharmacy support provided to specialist areas and identify opportunities for the pharmacy team to enhance patient care. This may include:

o Planned and unplanned surgery pathway development, in conjunction with SWLEOC and trust colleagues o Opportunities identified through close working with the Critical care MDT and governance structure

o Providing pharmaceutical expertise and support to clinic areas such as Endoscopy, Theatres and Pain Clinic

o Manage and further develop pharmacist independent prescribing within the division, to support the safe and effective use of medicines

22. To attend the Trust nutrition round and work with the other members of the Nutrition Team to provide advice on appropriate parenteral nutrition, and order according to individual patient need
