This role involves working closely with our partners across health and community services to build strong collaborative relationships that positively influence individual health and care outcomes.

Each team member maintains links with all three area clusters, ensuring a cohesive and integrated approach to service delivery. This means that the suitable individual will need to be able complete visits across the county. The position is based at county hall, Dorchester and office-based working alongside WFH can be supported
