Summary

The post holder will support the delivery of transition services in line with national best practice guidance, including: NHS England's Transition from Children's to Adult Services for Young People Using Health or Social Care Services guidance. NICE Guideline NG43: Transition from Children's to Adults' Services for Young People Using Health or Social Care Services. NCEPOD recommendations relating to transition, continuity of care, multidisciplinary working and the management of vulnerable young people with complex healthcare needs. The Care Quality Commission (CQC) expectations regarding person-centred transition planning and safeguarding. Relevant safeguarding legislation, including the Children Act 1989 and 2004, Care Act 2014, Mental Capacity Act 2005 and Working Together to Safeguard Children.
