Summary

RESPONSIBLE TO - REGISTERED MANAGER

PURPOSE OF POSITION

As a Head of Care, the role is to be part of and to support the Management Team in all aspects of the home’s management, including taking temporary complete charge during the absence of the manager or of a particular section of the home.

To provide leadership to Senior and care staff, in ensuring they are meeting their requirements in directing and training and mentoring the care staff to ensure personal care needs of the service users are met to a high standard, their wishes and choices are being adhered to End of Life Care plans are completed to a high standard.

To help train “On the Job” new and inexperienced members of staff in all aspects of their work.

To assist senior care staff to instruct care staff on electronic devices .

To induct Agency staff and ensure they are aware of the Electronic Devices and the processes and procedures of the home.

To ensure Safeguarding of residents at all times, and company procedures and policies are being adhered to.

To assist the management team to investigate incidents and accidents, assist with completing safeguarding referrals and notification to CQC.

To represent the company in meetings with other agencies or professionals as a when required, or delegated by the home manager.

PRINCIPLE RESPONSIBILITIES

· To develop care plans for all new residents, ensuring the support needs/requirements are met.

· To update care plans and risk assessments monthly along with the Senior staff to support

· To ensure Monthly weights are being completed on the same day each month

· To ensure all wounds, Body Maps and Infections are being dealt with efficiently and accurately recorded

· To liaise with social workers, DOLS Team, District Nurses and implement any risk assessments that are required to meet the needs of the residents

· To ensure all Supervisions of Senior staff are completed every 2 months

· To Supervise and lead the Seniors and care staff in supporting service users with mobility problems and other physical disabilities such as incontinence

· Completing Incontinence assessments

· Ensure MCM is being monitored throughout the day and Seniors and care staff are made aware of any requirements

· Contact Falls Team, SALTS and any other Health Care Professionals when required and to ensure accurate recordings and communication

· Ensure Activities are being completed throughout the home and the Senior is monitoring this

· Ensure Good Communication with all health Care professionals to include GP and Pharmacy

· To ensure Families are kept informed of all aspects of the service users’ lives, incidents, accidents, emergency services, doctors rounds and any other issues or concerns

· To ensure Family reviews are being completed

· To ensure Information in Residents rooms are updated as required

· To ensure Senior Audits are being completed and Completing Audits as requested by the Registered Manager or Deputy Manager

· Ensure meal times are engaging, atmospheric and documentation is accurate.

· Ensure all residents on Supplements are getting their nutritional needs met

· Ensure service users are looking their best at all times

· Ensure Senior Competency assessments are being completed.

· Ensure All service users have a copy of their care plan if they wish, they are in their own language and they are signing their Care Plan documents each month to say they are happy with their Care Plans and Risk Assessments.

· Best Interest meetings are being completed.

· Take part in Staff and residents’ meetings and in training activities as required.

· Monitoring the resident’s nutritional and Hydration needs.

· Delivering hands on care when required.

· Create, Manage and amend risk assessments, both at a strategic and operational level and to ensure that staff are following procedures.

· Act as a role Model for all staff, demonstrating and ensuring professionalism, good manners at all times.

· To Complete all tasks requested of you by your Registered Manager or Deputy Manager
