Summary
Key Responsibilities 1. Clinical Practice & Home Visiting Autonomous Consultations: Manage a clinical caseload using face-to-face, telephone, and digital triage pathways. Domiciliary Care: Undertake acute and routine home visits for housebound or complex patients. Clinical Assessment: Conduct detailed history-taking and physical examinations of undifferentiated conditions. Diagnosis & Management: Formulate accurate diagnosis and treatment plans for acute illness and chronic diseases. Independent Prescribing: Prescribe safely and effectively within current legal and local frameworks. Investigations: Order, interpret, and action diagnostic tests including pathology and radiology. Escalation: Make referrals to secondary care, community teams, or arrange emergency admissions when required. 2. Care Quality & Administration Documentation: Maintain accurate, comprehensive electronic patient records using EMIS Web. Coding: Ensure effective use of SNOMED CT coding for clinical information. QOF & Targets: Support the practice in hitting targets related to QOF and local health contracts. Audits: Participate in and lead clinical audits to maintain evidence-based local guidelines. 3. Leadership & Education MDT Collaboration: Work with GPs, Outreach Team and District Nurses, to coordinate care pathways. Mentorship: Provide clinical supervision and support to practice nurses and junior staff. Professional Development: Maintain professional revalidation in line with NMC standards.
