Summary
This role helps people recovering from stroke by providing rehabilitation support, making a real difference to independence and wellbeing.
You’ll work as part of a friendly, skilled community stroke team, supporting patients across their recovery journey. You’ll deliver care guided by Physiotherapy, Occupational Therapy, Speech and Language Therapy, and Nursing colleagues. Your work will help people regain skills, confidence and quality of life after stroke, both at home and in community settings.
Core Hours are 0800 to 1800, with a typical working day being 0800 to 1600 Monday to Friday, however, there is the opportunity to work condensed hours to enable a more flexible working pattern. Weekend working is a part of the role and usually works out around 1 in 8 weekends

What you’ll do:
• Support patient assessment and deliver agreed rehabilitation programmes
• Work with individuals and groups to promote independence and healthy living
• Monitor progress and report any changes to registered professionals
• Keep clear, accurate patient records in line with standards
• Support and guide support workers and new team members
What we’re looking for:
• A Level 4 qualification in health and social care, or similar experience
• Experience in a care or rehabilitation setting
• Good communication skills, with empathy and respect for others
• Ability to work independently and as part of a team
• Understanding of safe practice and when to escalate concerns
