We are CCP, an independent charity of around 250 staff and 100 volunteers across multiple counties and our purpose is to prevent
the causes and reduce the effects of homelessness, family breakdown and exclusion.
Our team is expanding, and we have a new opportunity that might be suited to you.
The role of a Children & Young People Social Prescribing Link Worker is to provide a diverse range of support services to
children, young people and families who are referred to the service through GP Practices. The post holder will empower individuals
to take control of their health and wellbeing through referral to non-medical ‘link workers’ who give time, focus on ‘what matters
to me’ and take a holistic approach, connecting people to community groups and statutory services for practical and emotional
support. The role will work across the TWNS PCN. This role will primarily be based in the Newent and Staunton areas.
Duties include:
Taking referrals from health care professionals and referrals from a wide range of agencies, working with GP practices,
pharmacies, multi-disciplinary teams, hospital discharge teams, allied health professionals, police, job centres, social care
services, housing associations, and voluntary, community and social enterprise (VCSE) organisations.
Working within communities, to establish needs based on a whole family assessment and contribute to forming a robust action plan
agreed with the family and other key agencies.
Assisting individuals to take decisions on what type of support they require that best meets their health and wellbeing needs.
This will involve using a range of behavioural change techniques such as; motivational interviewing, health coaching and
strength-based approaches to determine their skills, interests and motivation.
Co-producing and implementing strength-based Personal Development Plans & Family Plans based on their priorities, interests,
values and motivations – including what they can expect from the groups, activities and services they are being connected to and
what they can do for themselves to improve their health and wellbeing.
Providing a range of interventions that support the agreed outcomes such as parenting skills, behavioural, anger management and
social isolation.
You'll need to be a car driver with access to your own car.

Position: Social Prescribing Link Worker - CYP (Job ID 1108)
Hours: 22.5-26.5 hours per week, exact days to be discussed at interview
Contract: Fixed term until 31st March 2027 with the possibility to extend.
Location: TWNS PCN
Pay: £27,500 per year

^Annual Leave entitlement will be pro-rata for Part-time staff. Working on public holidays due to a rota pattern will be
compensated with time off in lieu. An extra day of leave accrues for each completed financial year (capped at 5). Purchasing
additional holidays is capped at 5 per year.
^^Contracted staff only. Business mileage (45p a mile) is paid for any daily miles over and above the distance from your home to
the allocated workplace location and back. You'll need a well-maintained vehicle with valid tax, MOT and Insurance covering
business use.
