Summary

This is an exciting opportunity to work in the Department of Acute General Medicine (AGM) at the Horton Hospital in Banbury, which offers excellent training in the management of patients on theOak Ward – HCU (High Care Unit), inpatient medical and trauma wards, the acute unselected medical take and the delivery of ambulatory care on the Rowan Ambulatory Unit (RAU).

In the event of sickness, you must inform the Medical Staffing Hub for Acute General Medicine (AGM), in the first instance, within one hour of the start of your duty period. At weekends, you must notify the Consultant on call for the day.

This post is not recognized for postgraduate medical training but will allow competencies to be gained that may count towards portfolios. Applications will be considered from less than full-time trainees.

Attendance at ward rounds, the day-to-day care of patients in wards and outpatient clinics, as appropriate to the grade of the post and under the direction of the responsible consultants. The post holder can expect to acquire practical experience and a knowledge of the diagnosis and management of selected common medical emergencies. Some examples listed below:

• the management of acute coronary syndromes, cardiac failure and dysrhythmias

• the assessment and management of the unconscious patient

• the assessment and management of gastrointestinal haemorrhage

• the assessment and management of respiratory infections and respiratory failure

• the assessment and management of frailty presentations including delirium

• the assessment and management of self harm patients

• the assessment and management of Diabetic Ketoacidosis

The Horton is a District General Hospital in Banbury with an Emergency Dept, admitting an unselected medical take.

There is a team of at least 8 general physicians based at the Horton operating a team-based structure, with on-call currently 1 in 8. We maintain the connection between the OUHFT hospital sites by having rotating posts and specialist input from the John Radcliffe Hospital to patients at the Horton. A consultant is typically supported by a team of 1 specialty registrar and 2 junior trainees – FY1 plus FY2 / CMT / GPVTS.

The wards consist of 2 large medical wards (approx. 28-30 beds each) and an EAU (Emergency Assessment Unit) together with a co-located short stay ward. Patients admitted to CCU are shared out to the medical teams onsite with support from the on-site anaesthetic team. We have developed a CCU ward-based team in addition to the other medical teams to allow for cohesive working on the critical care unit with the nursing and therapy teams there.

The Medical Take is unselected acute medical admissions; there are around 25/day. The consultant on call is responsible for the full 24hr period encompassing 3 hot rounds during the day (late morning, late afternoon and evening) and a follow up round in the morning for the overnight admissions. There is a 9am handover to the wards and the next on call team.

The post holder is responsible for ensuring that all duties and responsibilities of this post are conducted in compliance with the Health & Safety at Work Act 1974, Statutory Regulations and Trust Policies and Procedures. This will be supported by the provision of training and specialist advice where needed.
