Summary
A elderly lady from Glanamman requires a PA for 4 hours per week split between Tues & Thurs. Her first language is Welsh, she is happy to converse in both the Welsh and English language.
She requires support with her personal care, she enjoys having a shower, however Personal Assistant to be mindful that she does not like water over her face. Personal Assistant to provide a lady a choice of her preferred method of washing for that day. (Strip wash or shower).
Prescribed creams to be applied as directed by the Pharmacy if required.
Personal Assistant to monitor her skin integrity, any concerns to be reported to the most appropriate health care professional.
She requires support with dressing, carers to allow her choice.
Personal Assistant to prepare and serve meals and fluids of choice. She requires encouragement to drink enough fluids. At the time of this care and support plan she did not disclose any concerns regarding swallowing, there are also no known allergies.
Personal Assistant to ensure that she is wearing her lifeline at all times.
Personal Assistant to ensure any used areas are clean and tidy before departure.
Personal Assistant to ensure that all tasks and hours are documented accurately.
Principal duties and responsibilities:
1. Supporting with personal care
2. Supporting the employer with encouragement in gaining independence
3. Supporting the employer to develop independent living skills
4. To provide companionship in the employer’s home, including engaging in conversation with the employer and providing them with social interaction
5. Encouraging the employer to attend appointments
6. Ensuring the employer’s safety at all times during your working hours
7. To carry out any other reasonable tasks that are requested by the employer during your working hours
8. To ensure compliance with Data Protection Act.
9. Be mindful of safeguarding issues, and feedback any concerns to Carmarthenshire County Council immediately on (01267 228751)
10. Be responsible for contacting the GP, Social Worker or Local Authority should the employer need medical assistance.

1. Ability to develop supportive and caring relationship with employer.
2. To undertake all duties in a friendly and helpful manner.
3. Able to contact people on behalf of the employer when necessary.
Job Working Environment:
Working in employer’s homes and supporting them to access facilities and community activities.
