The post would suit a trainee in Surgery with an interest in mainstream surgical specialities. Ideally the trainee will have completed core training and will have passed or be studying for MRCS, but trainees having only completed foundation training will also be considered. This post will provide a solid basis from which to apply for core training or specialist ST3+ programs.

General Aspects

This post will rotate through Colorectal, Intestinal Failure, HPB and Transplant departments. The individual will work on the F2/CST tier and as such be expected to perform duties and take clinical responsibility to this level, as well as provide supervision as appropriate to foundation doctors.

Specific components of this post

The individual will contribute to the on-call rota for specialist tertiary departments (HPB/Transplant/Specialist Colorectal and IF surgery). Electively, they will contribute to providing high level care to complex inpatients including supervision of foundation doctors. The individual will also have opportunities to participate in theatre – as assistant in major resectional, transplantation and and also in daycase general surgery lists.

Teaching

There is no formal teaching post but this is expected to be added in 2028.

Personal Education

All staff are encouraged to question and learn from the extensive clinical activities that take place on the unit. There are a number of weekly meetings that provide excellent educational opportunities. These include; Directorate Audit/M and M meetings, MDTs and departmental meetings including research and audit updates. There is a regional SpR teaching programme at which attendance is encouraged.

Research/Audit

This directorate has a strong research pedigree and many ongoing clinical and scientific research projects. The high volume of specialist activity performed in the directorate ensures there is a vast opportunity to instigate and perform research/audit projects. Such activity is encouraged and results presented at regional, national and international meetings.

Clinical

Each doctor is expected to assist in the care of all patients to which he/she is allocated under the direct supervision of the Consultant. In addition, he/she is expected to provide ‘cross-cover’ for all patients in the Unit when required to do so. Duties include business, post-take and teaching ward rounds, OP clinics, Operating Theatre sessions, administrative (dictation of summaries, letters, coding etc.) and presentations (M + M, transplant assessment meetings etc.)

Administration

Each Junior Doctor is expected to carry out all administrative tasks allocated to him/her by the Consultants.

Out-of-Hours Commitment/Pay

The post holder’s out-of-hours contribution commitment will be to the Surgical SHO rota, approximately at 1:7 frequency including day and night shifts. The enhancements will be confirmed at the offer stage.

Emergency and On-Call responsibilities

The doctor accepts that he will also perform duties in occasional emergenciesand unforeseen circumstances at the request of the appropriate consultant in consultation where practicable with his colleagues both senior and junior. It has been agreed between the professions and the Department that while juniors accept that they will perform such duties, the Secretary of State stresses that additional commitments arising under the sub-section are exceptional and in particular that juniors should not be required to undertake work of this kind for prolonged periods or on a regular basis.

Colorectal Surgery

The Freeman Colorectal Unit has four Colorectal Surgeons, a Royal College Of Surgeons Senior Robotic and Laparoscopic Fellow, one Trust Junior Fellow (ST1-CT2 equivalent, this post) and two FY1s. The Freeman Hospital provides many highly specialised services which attract unusually complex patients. The Colorectal Unit covers all aspects of elective Colorectal Surgery, including Robotic and Laparoscopic, Malignant and Benign disease, and abdominal wall surgery. In addition, there is overlap with the IF unit, sarcoma service (pelvic sarcomas) and other tertiary specialist services requiring joint surgery eg HPB, Urology.

Accident and emergency is at the RVI where most emergencies Initially are admitted, there is also a steady flow of urgent patients at the Freeman, often with complex multidisciplinary problems. The Trust Fellow also participates in the RSO rota and will be involved in acute HPB emergencies.

This job provides plenty of opportunity to develop clinical and surgical skills and mature as a junior surgeon. The Colorectal MDT is cross-site and busy, providing expert training opportunities in cancer assessment management. The unit is heavily involved with the Newcastle Surgical Training Centre and the candidate will be encouraged to participate in audit projects.

Intestinal Failure Unit

The Nationally designated Intestinal Failure Unit provides comprehensive care for patients with intestinal failure and complex gastrointestinal conditions and complications. There is a significant component of inflammatory bowel disease. This post is suitable for a junior surgeon with an interest in the surgical aspects of gastrointestinal patients. Care is provided in conjunction with a diverse multi-disciplinary team of specialists, including Colorectal Surgeons, Gastroenterologists, Parental Nutrition Specialist Nurses, Stoma Specialist Nurses, Dieticians, Pharmacists, Therapists, etc. The job entails providing ward based care, attending theatre lists, and surgical outpatient clinics. There is cross cover with the designated Colorectal Junior Fellow, for ward care and theatre lists. On call duties allow exposure to some emergency surgery which trainees value. Successful applicants will have ample opportunities to progress their surgical logbook with interesting cases such as robotic colorectal surgery and complex laparotomies for enterocutaneous fistulas.

HPB / Transplant Surgery

This is run as two interlinked areas, HPB and transplantation. The unit provides secondary, tertiary and superregional care. There is plenty of core surgical activity to interest core trainees, and also the opportunity to see high volume complex cases which is unique to the North East of England. There is also a third major interest in soft tissue Sarcoma Surgery.

The transplant team specialise in kidney, liver and pancreas, and Newcastle is one of the active designated Assessment and Recovery Centres (ARC). There is a separate Retrieval Team which travels across a wide geographical area and participates in the National Organ Sharing Scheme.

The successful candidate will be well supported by a large multidisciplinary team which includes senior HPB and Transplant fellows, Associate Physicians, Specialist Nurses, and many collaborative departments such as Interventional Radiologists and Intensivists. The unit has a strong academic profile and encourages trainees to get involved.

Flexibility:

In line with the Trust’s core value of placing patients at the heart of everything we do, we are developing our service provision to be responsive to the needs of our patients. To meet those needs some staff groups will be increasingly asked to work a more flexible work pattern so that they can offer services in the evening or weekend.
