Mental health practitioners play a key role in dissolving the barriers that can exist between mental health and physical health, social care, voluntary and community social enterprise organisations and local communities, and primary and secondary care, to deliver integrated, personalised, place-based and well-coordinated care for adults – moving towards a ‘whole system’ of mental health care, which will provide timely access to support, care and treatment. We need qualified, experienced, skilled and professionally registered practitioners, to support people with mental health needs to access mental health care when and where they need it, and to be able to move through the system easily. What we are looking for are highly motivated and enthusiastic practitioners with a passion for psychosocial rehabilitation who are excited by service development. You will need experience at band 5 (or equivalent) and of working in the communityto be able to step up to this band 6 role. Depending on your professional background, this role may include clinical assessment and evidence-based interventions, coordination of mental and physical healthcare, relapse prevention and recovery-focused support, alongside the psycho-social interventions that help people manage their problems and move towards individualised recovery on their own terms – surrounded by their families, carers and social networks, and supported in their local community. You will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of service users experiencing complex psychosis. You will support individuals to be able to contribute to and be participants in the communities that sustain them. A key part of the role will be liaison with Local Authority and ICB colleagues to ensure that any out-of-area and supported accommodation placements are suitable for the needs of the service user and that people are supported in the least restrictive environment possible. This will involve: critically evaluating and interpreting patient information (prescription charts, patient notes, observation charts, laboratory results and medicines) to ensure that prescribed treatment is safe, effective and appropriate for the patient proactively suggesting evidence-based improvements to drug therapy and management of patients to consultants and senior medical staff, anticipating a positive outcome for the patient providing evaluated information to consultants, senior medical staff, nursing staff, pharmacists and other health care professionals, making recommendations where opinions differ and information is lacking counselling patients, relatives and carers about medication, checking their understanding of information and being mindful of physical and language barriers promoting cost-effective prescribing by encouraging and supporting compliance with the Formulary and by advising medical, nursing and other healthcare professionals including pharmacy staff, in line with Trust, local and national policies and procedures feeding back to the Head of Pharmacy and other members of the team, changes that impact on current practice. To dispense/supply medicines safely and legally for all patients. To manage the pharmaceutical care needs of patients at admission through to discharge using the staff in the team. To respond appropriately to requests for information on medicines from patients and/or their representatives to promote understanding of medicines to ensure compliance and concordance. To respond appropriately to requests from all levels of medical and nursing staff and other healthcare professionals for information and advice about the selection and use of drug therapy and to promote the cost-effective use of drugs. To be responsible for the proposal, preparation and maintenance of written policies and procedures relating to pharmacy matters within the Division. To assist in the development of standards of practice for the service/team and to review its performance. To be responsible for ensuring regular stock list reviews in liaison with the ward or department managers/lead nurses and appropriate pharmacy staff. To ensure that a record of all interventions/contributions to care is maintained and audited at intervals as specified by the Principal Pharmacist. To identify adverse drug reactions and potential defective medicines and to report these according to local and national guidelines. To contribute and lead on the development of clinical guidelines, PGDs, protocols and procedures within the Division. To attend relevant external peer group networks and report back appropriately. To act as a role model in promoting good professional practice and to be an ambassador for Pharmacy. To demonstrate current Continuing Professional Development and participate in the Pharmacy departments education and training programme. To support formulary and Principal pharmacist in managing drug shortages. To act as an Independent Prescriber, when applicable to patients care in accordance with the Trusts Non-Medical Prescribing policy. Managerial To plan and prioritise personal work and work of a designated team to meet agreed deadlines for completion of duties. To monitor and maintain performance of pharmacy staff and the efficiency of the service/team against standards and key performance indicators. To provide explanation of variations in drug expenditure and to assist in monitoring expenditure and financial planning. To investigate untoward drug incidents and complaints, action any corrective procedures necessary and report back via governance structures as appropriate. To apply Risk Management principles to the practice of Pharmacy, support the departments Risk Management strategy and actively manage risk relating to medicines in the Division. To contribute to the clinical governance plan for the clinical specialities and Pharmacy department. To formulate strategies relating to optimisation of medicines and pharmacy services in response to development and changes within areas of responsibility. This includes contribution to annual business planning and relevant business cases. Service provision To have an overview of all aspects of the service provided by the department to ensure continuity of care and maintenance of professional standards. To ensure that all activities undertaken contribute wherever possible, to an accurate patient record, electronic or paper. To plan, organise and participate in clinical audit, drug use review, practice evaluation and development and project work as dictated by service needs. To advise the Principal Pharmacists on developments and new initiatives and to plan their implementation in consultation with the Principal Pharmacists. To participate in clinical trials, research and audit where appropriate. To actively contribute to multidisciplinary meetings and ward rounds in consultation with the Principal Pharmacists. To provide and interpret information relating to service delivery (eg workload statistics), drug utilisation and expenditure. To ensure safe systems of work in line with current Health and Safety policies. To observe departmental and Trust policies and procedures and report deviations from them to appropriate staff, resolve these issues and feedback non-compliance to appropriate staff. To deputise for the Principal Pharmacists and cover the absence of other Senior Pharmacists when required. To liaise with other senior Pharmacy staff in promoting the efficient use of staff. To lead and facilitate pharmacist peer review meetings. To assist in the recruitment of staff when required. To participate in the staff appraisal system and undertake appraisal of staff/ To participate in the Emergency Duty Commitment. To participate in extended hours/flexible working as required by the service, including 7-day service requirements. To carry out any other duties as reasonably assigned by the Head of Pharmacy. Staff Development Provide effective leadership to all staff and contribute to their leadership development. To provide supervision and in-service training for all pharmacists, pharmacy technicians and other pharmacy staff. To identify training needs and agree personal and professional development plans for staff in the speciality. To participate in the formal service needs training for pharmacists, pharmacy technicians, pharmacy assistants and other pharmacy staff in the capacity of tutor or mentor. To be responsible for the training of rotational pharmacists, trainee pharmacists, pharmacy technicians and other pharmacy staff in the performance of their duties.
