They will facilitate the achievement of person-centred outcomes for people living in the community, some of whom have complex mental health and other support needs. Support workers will work collaboratively with professionals, including care coordinators, social workers, occupational therapists, psychologists and psychiatrists, as detailed in their support plans to achieve agreed outcomes and provide best practice support for clients. The support worker will act as ‘social prescribers’, connecting people to local groups and services including peer support. They will also help people reconnect with family and friends.
