The MDT Co-ordinator role aims to ensure the smooth running of the MDT case conferences by ensuring the MDT meetings run effectively and are well structured. All patients are treated and discussed according to evidence-based protocols. While providing an efficient and confidential administrative service to the Proactive team & Community Matron Team. By liaising and networking with MDT members and Enfield GPs, the post holder will promptly ensure that patients are referred to the Enfield MDT, enabling the patient to be discussed and referred to the appropriate team. The post holder is responsible for capturing all patient information, ensuring that up-to-date, relevant information is recorded on the database, and providing feedback to the referring services. The post holder will coordinate cross-site MDT meetings, ensuring the effective running and structure, ensuring all necessary information is available to enable clinical decisions, and ensuring that decisions are documented according to local protocols. Ensuring patients’ diagnoses, investigations, and management and treatment plans are completed and added to the patient’s notes. • Ensure there is a clear definition of roles and responsibilities for MDT members. • Where appropriate, ensure there is a room and telephone available in a confidential area if dialling in from various locations is required. • To be responsible for the collation, collection and inputting of the patient activity data required to monitor and quality assure the Enfield Integrated Locality Team Services. • To track the actions on an Excel tracker and follow up with the owners of the actions to track progress. • To maintain a history of the patient information provided to the MDT. • To support the rollout of the risk stratification tool to practices, completing any evaluation and assessment required. • To provide information to finance to verify that individual patient care plans have been created and payments are due. • To assist in the production of reports as required, e.g. tracking case conference outcomes. • To organise any other MDT meetings as required, e.g. review/evaluation meetings. • To escalate any issues to the Chair/Project manager as appropriate • To ensure own actions contribute to the maintenance of a quality service provision. • To be cognisant of IG requirements and provide advice and support to MDT participants to be compliant. • To provide input into the development of the OPAU. • The post will be regularly reviewed, and additional duties may be assigned depending on how the post develops. • Ensure telephones are answered promptly to facilitate efficient communication during office hours. • To type and distribute clinical and general correspondence per Trust standards. • To have responsibility for input and maintenance of good quality data. • To organise and have responsibility for all communication hardware and software agreed with the lead members of the MDT. The Neuropsychiatry Service, based at St George’s Hospital is a regional outpatient service that provides assessment and treatment for adults over the age of 18 with neurological illness or head injuries who have associated psychiatric problems. Acting as the central hub of the department, the coordinator ensures that cardiac diagnostic and interventional procedure lists run seamlessly, patients are scheduled accurately. Utilizing hospital computer systems such as Cerner, to log patient outcomes, open or close treatment access plans, and update medical electronic records. Communicating constantly with cardiologists, nurses, radiographers, and cardiac physiologists to align staff availability with scheduled lists. Acting as a welcoming and empathetic face or voice for patients, relatives, and carers who may be highly anxious about their upcoming heart procedures. Managing patient cancellations at short notice and proactively filling vacant slots to ensure the lab operates at full capacity.
