This service supports individuals aged 18 to 65 who are facing their first episode of psychosis. In the context of Westminster Perinatal Mental Health Services, postholders will have experienced recovery from perinatal mental health challenges and accessed support through a perinatal mental health service in relation to this. Through sharing wisdom from their own experiences, Perinatal SpPSW inspire hope and belief that recovery is possible in others. Using recovery principles and socially inclusive practice, the post-holder will support service users in their personal recovery journeys through the Perinatal Mental Health Service. This means providing peer support for mums who experience similar mental health and social needs during the perinatal period. As an integral and highly valued member of the multi-disciplinary team, the Addictions Clinical Support Worker will be able to provide drug and alcohol assessments, brief interventions, prepare prescriptions for printing and recording on FP10 records. Participating in the care planning, provision and evaluation of patients’ needs under the supervision of trained staff. The clinical support worker will take a lead role in embedding recovery values within the service setting in which they work, alongside peer support workers from other agencies in Addictions and act as an ambassador of recovery for the Addictions Directorate with external agencies and partner organisations. The clinical support worker will promote their role, through the provision of information and a range of Learning opportunities for the multidisciplinary teams and others across the Addictions Directorate. A valuable substantive job opportunity has arisen for a highly motivated band 6 practitioner (RMN) to work within the team. This is an innovative service commissioned for Merton residents, serving as the primary access point for mental health referrals into secondary care. The post holder will be responsible for conducting mental health assessments for people who are referred by a variety of agencies, including self-referrals, and the post holder will contribute to the triaging of these cases by conducting risk assessments by variable forms of communication including telephone, e-consultation and face to face review. These assessments will be conducted at the Wilson Hospital site and variable community settings such as patient homes depending on the nature of need identified during the triage process. The post holder will also be responsible for identifying safeguarding concerns, and undertaking plans of action to help mitigate/reduce identified safeguarding risks. The post holder will carry an assessment caseload and be responsible for the management and planning of those service users, under the supervision of the Team Manager and Consultant Psychiatrist, including discharge to alternative primary care services, or onward referral to secondary care or specialist teams within the trust. The Post holder will also require a degree of autonomy, and robust assessment/formulating skills are essential.
