This role is part of a new and exciting service providing forensic community service to a specialist group who require expert interventions. The FIND service works with adults with a diagnosis of a learning disability (LD) and/or Autism (ASD) who also experience additional mental illness, co-morbid neurodevelopmental conditions, or behaviours which challenge and who have had contact with forensic services. The team covers the London boroughs of Wandsworth, Sutton, Merton, Kingston and Richmond. The team is informed by principles of assertive outreach and largely follows the PACT model. The team is multidisciplinary and currently comprises of a Psychiatrist, Team Manager, Psychologist, Social Worker, Speech & Language Therapist, Occupational Therapist, Community Psychiatric Nurses, Assistant Psychologist and an Office Manager. Patients are seen at their homes and hostels and at a variety of community locations, and the job therefore involves a considerable amount of travel. The post holder will be responsible to the Team Manager and will receive professional supervision from the senior Social Worker. The successful candidate will work within the Forensic service, providing support to individuals with learning disabilities and/or autism leaving secure mental health services across the South London Partnership (SLP) and those in the community. Covering the central London Borough of Camden & Islington, the team offers an intensive crisis assessment, resolution and home treatment service providing mental health crisis care to our local residents age 18 and upwards. Crisis home treatment promotes social inclusion and also supports the social network in caring for the individual in the least restrictive environment. The Crisis Team acts as an alternative to hospital admission. In order to fulfill this role, the team provides a fully operational 24 hour service, 365 days per year. Successful applicants will be required to have proven and post qualification experience with desirable acute setting experience . You must be willing and committed to working as part of a 24-hour rota to include weekends. As most of our service user contact takes place within their own home, you must also be able to either drive (we utilise Trust vehicles) or to be able to travel independently via public transport. The qualified Education and Mental Health Practitioners (EMHPs) or Children’s Wellbeing Practitioners (CWPs) or will join this growing team (that includes CAMHS clinicians, Educational psychologists, and trainee EMHPs) to support the work within schools. The post holder will providesupport for children and adolescents with mild moderate emotional and mental health difficulties and deliver evidence-based 1:1 and group interventions in schools, employing CYP-IAPT principles (Children and Young Peoples Improving Access to Psychological Therapies). The post holder will develop relationships with designated mental health leads in schools, alongside CAMHS clinicians / supervisors, and educational psychologists, and help them develop their approaches for mental health and wellbeing in schools. They will gain some experience of teaching, training and consultation through this joint working. 1. Contribute to the assessment, and deliver outcome focused, evidence-based interventions 2. Work in partnership with children and families 3. Support and empower children, young people and families to make informed choices about intervention. 4.
